| Objective : Chronic obstructive pulmonary disease(Chronic obstructive pulmonary disease,COPD)is a kind of common disease of respiratory system,which is one of the main causes of death of our patient population.In recent years,found that the acute exacerbation of chronic obstructive pulmonary disease(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)in serum of patients with type B natriuretic peptide(B-type natriuretic p Eptide,BNP)and high sensitivity C-reactive protein(hypersensitive-CRP,hs-CRP)between the possible link.This paper aims to investigate the level of phase and stable phase BNP and hs-CRP in serum of patients with acute exacerbation of COPD,and the relationship between value BNP and hs-CRP,through statistical analysis,to explore the significance of BNP and hs-CRP in clinical diagnosis of COPD disease the severity and guiding treatment.Method:Choose 70 cases of acute exacerbation of COPD in our hospital from March2015 to October 2016 in the year.For A group,hospitalization or outpatient follow-up of COPD patients with 24 cases of B group,30 cases of health examination in our hospital.The BNP and hs-CRP levels of the three groups were measured and compared with those of the control group(C),and the serum BNP levels of each group were compared.And hs-CRP level difference;secondly,the correlation of BNP,hs-CRP and GOLD grade in A group were analyzed.The role of A in response to disease severity was analyzed.Finally,Finally,The change of BNP,CRP and EF were compared before and after treatment.Result:(1)The BNP level of A patients before treatment in group AECOPD was 180.44 +75.79pg/ml,The hs-CRP level was 38.97 + 18.21mg/L,and the data of the were the highest(P<0.01)between the three groups(B).The detection of BNP and hs-CRP in patients with stage COPD was 84.70 + 14.28pg/ml,+ 17.63 + 6.23mg/L,The C group,the serum BNP,hs-CRP concentration was lowest in the three groups,BNP=39.17 +10.15pg/ml,Hs-CRP= 1.30 + 0.47mg/L;these differences were statistically significant (P<0.01)(2)Pearson correlation analysis showed that the serum BNP and hs-CRP of the patients with COPD in 94 cases were linear.Correlation and positive correlation(r=0.825,P<0.001),the regression equation was y(BNP)=39.647+3.471x(CRP).(3)according to GOLD classification,the levels of BNP,hs-CRP and GOLD in patients with AECOPD in 70 cases.Pearson showed a positive correlation with BNP score and GOLD score(r=0.581,P<0.01,r=0.401,P<0.01)in patients with AECOPD(4)the serum BNP and hs-CRP levels in group A were significantly higher than those before treatment.And the difference was statistically significant(P<0.01),The values of EF before and after treatment were59.30 + 10.74%,62.40 + 10.35%,no significant difference before and after treatment(P=0.084)Conclusion:1.The levels of plasma BNP and hs-CRP in patients with AECOPD were higher than those in stable COPD patients and healthy subjects,and the two indexes were decreased after the acute phase treatment.These differences were statistically significant(P<0.01)2.The combined detection of BNP and hs-CRP has a certain significance in clinical disease severity in AECOPD and clinical treatment,the combined detection of BNP and hs-CRP can be considered as a judgment of the severity of COPD and guide the treatment without pulmonary function instrument conditions. |
