| ObjectiveTo investigate the connection and clinical significance between non-high-density lipoprotein cholesterol and homocysteine level with ischemic stroke in high altitude localities.Methods158 patients with ischemic stroke were treated as case group,which was collected from January 2016 to December 2016 in the Affiliated Yanan Hospital of Kunming Medical University At the same time,randomly selected normal people of the physical examlation center in 30 cases as control group.Based on the NIHSS,TOAST,aera of ischemic stroke,location of ischemic stroke ,hypressure and hyperuricemic,divided into two groups.While we collected the medical information of research subjects,including age,sex,blood pressure,fasting plasma glucose (FPG ),total cholesterol(TC),triglyceride(TG),high-density. lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol (LDL-C)levels and the history of taking lipid-lowering drugs,hypertension,diabetes,smoking.The differences of non-HDL-C and traditional ischemic stroke risk factors were compared between the case and the control group.The correlations of ischemic stroke with traditional risk factors and non-HDLC were assessed by bivariate correlation analysis.The association between non-HDLC and ischemic stroke were evaluated by logistic regression.Results1.The level of non-HDLC、LDLC、HDLC、TC、HCY、UA in the case group was higher than that of the control group. ( P <0.05 )2. The level of non-HDLC、HCY、UA in the hypertension group was higher than that of the normal blood pressure group. HCY ( mean±SD,15.89±0.06mmol/L VS 10.68±0.12mmol/L ) ,Non-HDLC ( mean±SD,3.20±0.08 VS 2.86±0.10 ) ,UA(mean±SD ,356.9±10.82 VS 302.5±11.0) (P<0.05)。3. The level of non-HDLC、HCY has no relation with the aera of ischemic stroke,and the location of ischemic stroke.4. The level of non-HDLC in the moderate-severe ischemic stroke was higher than that of the mild (3.41±0.11mmol/L VS 3.06±0.10mmol/L) , P<0.05.5. The level of non-HDLC in the LAA ischemic stroke was higher than that of the SAO and the non-LAA& SAO, (4.04±1.03 VS 2.98±0.64 VS 2.75±0.78),P<0.05.6. The level of non-HDLC in the high homocysteine group was higher than that of the normal homocysteine group. P<0.05.7.After adjustmen with other confounding facors,Logistic regression showed that non-HDLC , HCY were an independent risk factor for the ischemic strokein high altitude localities. (Non-HDL-C (OR 2.116,95%CI 1.173~4.051,P<0.05);HCY (OR 1.235,95%CI 1.000~1.525,P<0.05))ConclusionNon-HDLG,HCYmaybe an independent risk factor for the ischemic stroke in high altitude localities. The level of non-HDLC has relationship with the TOAST,the NIHSS ,HCY,hypertension,but no relationship with the aera of ischemic stroke,and the location of ischemic stroke. Non-HDLC probably maybe an independent risk factor for the LAA ischemic stroke,and also maybe an independent risk factor for ischemic stroke with hyperuricemic. |
