Multivariate Analysis Of Prognostic Factors In Middle Age Patients With Lung Cancer

ObjectiveTo retrospectively review the clinical characteristics and analyze the prognostic factors of 183 middle age patients(45~60 years old)with lung cancers.MethodsThe clinical data of 183 middle age patients with lung carcinoma in the 1st affiliate hospital of Dalian medical university gathered from March 1993 to March 2002 were retrospectively evaluated by univariate and multivariate analysis,including the gender,localization(central or peripheral),nodal status,histological subtype,invasion of pleura,cell differentiation and surgical methods.Kaplan-Meire method was used to calculate the survival rate,its significance was tested by log-rank test.Multivariate factors for survival were analyzed by Cox proportional hazards regression model.ResultsThe 1 year,3 year and 5 year survival rate of the 183 middle age patients was 76.0%,33.3%and 13.7%respectively.Univariate analysis revealed that the overall survival rate of different type of pathology was,73.8%for squamous carcinoma,75.5%for adenocarcinoma,40.0%for small cell carcinoma,50.0%for large cell carcinoma and 75.0%for others(p=0.022).The overall survival rate of patients without lymph node metastasis was 75.6%,while those with node metastasis was 67.3%(p=0.013).The overall survival rate of patients with pleural effusion was 65.5%,and witch is higher(75.6%)in patients without pleural effusion(p=0.015).The overall survival rate was 100%in stage 0,90.9%in stage IA,72.5%in stage ⅠB,80.8%in stage ⅡA,89.5%in stage ⅡB,63.6%in stage IIIA,46.2%in stage IIIB.The worst is stage IV,the overall survival was 37.5%(p<0.001).Different surgery also indicated different overall survival rate,which is 66.7%in local resection,78.2%in lobectomy,50.0%in pneumonectomy,and 50.0%in other methods(p=0.002).Without that,other characteristics,like gender(p=0.455),localization(p=0.744)and differentiation status(p=0.295)had no significance in the prognostic of lung cancer in middle age patients.The result of the Cox proportional hazards regression showed that the TNM staging(p=0.001)and the histological subtype(p=0.027)are confirmed to be the independent prognostic factors of the lung cancer in middle age patients.ConclusionThe TNM staging and the histological subtype are confirmed to be the independent prognostic factors of the lung cancer in middle age patients.ObjectiveTo investigate the expression of Caveolin-1 using the small hairpin RNA(shRNA)which targeting Y-box binding protein-1(YB-1)gene in lung adenocarcinoma cell line A549.Meanwhile,to observe if there any differences of the cells’ ability of migration,proliferation and invasion after inhibited the YB-1 in A549.MethodsDesign two strands of specific shRNA and one strand of negative control sequences in accordance with YB-1 mRNA and then transfected into A549 with lipofectin respectively.Reverse transcription-polymerase chain reaction(RT-PCR)and western blot were performed to analyze the mRNA and protein expression of YB-1 and Caveolin-1 in A549 cells.Cell migration,proliferation and invasion were assessed by wound-healing experiment,methyl thiazol tetrazolium(MTT)and Matrigel invasion assay.Results1.RT-PCR results showed that the expression of YB-1 and Caveolin-1 mRNA was significantly decreased in experiment groups than that in blank control group and negative control group(p<0.05).2.The results of western blot showed that the YB-1 and Caveolin-1 protein expressed in experiment groups significantly lower than in other groups(p<0.05).3.The proliferation of A549 cells was markedly inhibited by YB-1 shRNA with the inhibition rate at 37%and 28%(p<0.05),while scratch repair time extended.Meantime transwell cell invasion assay showed that the invasive ability of A549 cell was also inhibited in the experiment groups sh-1(35 ±5)and sh-2(23 ±3),compared with the blank control group(214±10)and the negative control(187 ±10)(p<0.01).Conclusion1.When the YB-1 was silenced by the YB-1 shRNA,the expression of Caveolin-1 was down-regulated.2.Down-regulation ofYB-1 can inhibit migration and invasion of A549 cells.
