The Clinical And Experimental Study On Jiawei Li Chang-yin In Treating Liver-stagnation And Spleen-deficiency IBS-D Visceral Hypersensitivity Based On The Method Of "Harmony"

Objective:This article is in the experiential prescription of Professor Chi Lili Jiawei Li Chang-yin in treating diarrhea type irritable bowel syndrome(IBS-D).Based on the previous research,to further evaluate the clinical curative effect of Jiawei Lichang Decoction on treating IBS-D from two aspects of clinical and animal experiment,and observe the effects of Jiawei Decoction on physical intestinal nitric oxide IBS-D in rat plasma 5-HT,SP,(NO)on the level of 5-in colon 5-hydroxytryptamine 3 receptor(5-HT3R)and neuropeptide mRNA(NPY)mRNA expression and the effects of changes in the number of mast cells,to observe the effect of Jiawei Decoction for the treatment of intestinal physical mechanism of deep IBS-D.Methods:1.Clinical research: will be included in the standard of 80 patients,by adopting the method of randomly divided into experimental group(40 cases)and control group(40 cases)and treatment group given Jiawei Li Chang-yin Chinese traditional medicine medicinal broth of oral,the control group gives the western medicine oral Dicetel,a course of 28 d.After treatment,the two groups were treated with total efficiency,syndromes of traditional Chinese medicine,the rate of adverse reaction and the recurrence rate after six months.2.Experimental study: the SPF 70 healthy SD rats were divided into blank control group,model control group,and Dicetel,Compound Glutamine Entero-soluble group,Jiawei Li Chang-yin low,medium and high dose group.In addition to the blank control group,each group to avoid water stress+clip tail stimulation,senna lavage method to establish the rat model with IBS-D,after the success of the building,respectively,to give corresponding drug lavage,Dicetel group with Dicetel 15mg/(kg/D)to fill the stomach,group 0.2 g/(kg/D)to fill the stomach,flavored reason im drinking low,medium and high dose group were given 1.5g/(kg/D),3 g/(kg/D),6g/(kg/D)herbal medicine by gastric juice,such as blank control group and model control group were given volume normal saline lavage,1/D,28 D continuous administration after the death of rat,fresh blood and colon specimens,by using Elisa method to detect the rat 5-HT,SP,the NO level in plasma,and the PCR method to detect 5-HT3 R mRNA,NPY mRNA expression,observing rats colon mast cells and pathological changes.Results:The Clinical part: jiawei Li Chang-yin in improving patients with abdominal pain and,then empress pain reduction,abdominal distension,defecate frequency,defecate character,on clinical symptom such as depression or irritability,loss of appetite is superior to the Dicetel group(P<0.05);Two groups in improving bowel vector,burnout lazy speech differences no statistical significance(P>0.05),and on the drug safety,efficient treatment,reduce the recurrence rate is better than Dicetel group.The experimental part: compared with the blank control group,the plasma 5-ht and SP expression of rats in the control group were raised(P<0.05),and the NO level decreased(P<0.05).In 5-HT,except jiawei Li Chang-yin low dose group,flavored principle,high dose group,im drinking GuCan intestinal Ann,and Dicetel group can make 5-HT levels significantly lowered(P<0.05 or P<0.01),among them,jiawei Li Chang-yin and high dose group is superior to other drug group(P<0.01),but no statistical significance between two groups.In SP,the SP level(P<0.05orP<0.01)was reduced in all drug groups except for the Dicetels group(P<0.05 or P<0.01),and the high dosage group was the best(P<0.01);In NO respect,except jiawei Li Chang-yin low dose group,the drug group,can raise the NO level(P<0.05 or P<0.01),especially in the jiawei Li Chang-yin dose group of the best effect(P<0.01);In the expression of 5-ht3 r mRNA,the dosage group in the flavoured bowel drink was significantly more significant than the other group in the other drug group(P<0.01).Each drug group all can increase the expression of mRNA,NPY of flavored reason im drinking,high dose group is superior to other drugs(P<0.01),but the comparison between the two groups had no significant difference(P> 0.05).The number of colon mast cells was reduced in the high-dose group and the glutamin group,especially the high-dose group.Conclusion:Flavored the visceral hypersensitivity;of intestinal drink of rats and intestinal dynamic anomaly has obvious effect,and the dose groups to improve the symptoms of rats,biological indicators are superior to Compound Glutamine Entero-soluble group,and Dicetel,its mechanism may be involved in regulating NPY,SP,NO,5-HT secretion and release,adjust the number of colon mast cells,reduce 5-HT3 R mRNA expression level,raise NPY mRNA expression to slow down the bowel movement,reduce intestinal inflammation,reduce their internal gao min,thereby reducing the abdominal pain diarrhea symptoms.Combined with experimental research and clinical research,it is a safe and effective prescription for IBS,which has important clinical value and theoretical value.
