Objective 1.At this stage,the curative effect of 1 stent assisted aneurysm and microsurgical treatment of complex aneurysms,analysis of strengths and weaknesses of.2.into two types and surgical treatment by comparative analysis and application evaluation of two type of therapy,provide a theoretical reference for the clinical treatment of patients with complex aneurysms.Methods In our hospital treatment of patients with complex aneurysms in recent years,through the assessment and postoperative visit,at the present stage evaluation of curative effect of stent assisted aneurysm and microsurgical treatment of complex aneurysms.Inclusioncriteria: 1 on t he basis of the head of CTA,MRA or DSA for the diagnosis of intracranial aneurysm;2 intracranial aneurysms diagnosed include aneurysm,wide necked and fusiform,and the characteristics of micro dissection,is characterized as complex intracranial aneurysm;the 3 aneurysms underwent microsurgical clipping or interventional embolization surgery.Exclusion criteria: 1 patients after 1 years without follow-up followup.2.after January were lost.By reviewing medical records of patients.A retrospective analysis of patients with basic information: gender,age,previous history;at the time of admission,WFNS score,GCS score;the number of the location of the aneurysm,and the size and type of surgery;operative time,perioperative treatment.Patients were followed up and the imaging data were collected.The CT and DSA data of the patients who were diagnosed with aneurysm and postoperative reexamination were collected to evaluate the recovery of the patientsResults In 187 cases of confirmed patients,74 cases with endovascular stent assisted therapy,113 with microsurgical treatment.The last time the average follow-up time was 179.6 days,average and median time on the vascular stent assisted treatment for the 203.9 cohort and 176.5 days in the surgical treatment of the queue for 154 days.In the age.Gender,WFNS classification and Fisher,median aneurysm size and length of stay in ICU.There is no difference between the aneurysm in the middle cerebral artery occupies a h igher proportion(37.1% vs.8.8%;P < 0.001),the proportion of the remaining positions in the aneurysm was lower(P < 0.001).94.7% aneurysm clipping 98.6% of the patients and stent assisted aneurysm embolization patients discharged home.The first and last follow-up improvement when the Rankin score had no significant difference.Most of the patients had no significant disability.The treatment cohort of endovascular surgery the mortality rate was 2.7%.The mortality rate was 5.3% in the closed cohort,and the mortality rate of the last follow-up was about 2.7%,and the rate of surgical clipping was about 6.2%(no significant difference).The conversion rate of endovascular treatment to surgical treatment was 4%.Conclusion The treatment of complicated aneurysms should be high body customization,with endovascular stent assisted treatment and surgical treatment for complementary means.Clinical characteristics should assess patients and complex aneurysms,based on the two kinds of treatment technology,choose the best treatment.
