Analysis Of Therapeutic Effect And Influencing Factors Of Gamma Knife In Brain Metastases Of Lung Cancer

Purpose:To investigate the effect of gamma knife in the treatment of brain metastases of lung cancer,and to analyze the related factors influencing the survival rate and provide evidences for clinical treatment.Method:This study retrospectively analyzed between January 2015 and December 2015 at the Beijing institute of neurosurgery gamma knife treatment center for gamma knife treatment of patients with brain metastases of lung cancer,the age of 36-84 years old(average 60.06 + /-10.73 years)pathological classification: non-small cell lung cancer: 107 cases(89.2%),9 cases of small cell lung cancer(7.5%),mixed lung cancer,4 cases(3.3%).Treatment program at the center of the dose of 20.0 36.0 Gy(28.90 + /-3.22 Gy),edge dose of 11.0-18.0 Gy(14.08 + /-1.45 Gy).By measuring the therapy localizer and follow-up MRI tumor size,tumor volume change before and after the comparison of gamma knife treatment;For group survival analysis using Kaplan Meier curve,chi square test was carried out on the categorical data of statistical significance.Results:This study included 120 cases with the follow-up from 3 to 24 months(mean 13.22 ± 6.76 months).At the end of the study,there were 73 death cases.19 of 73 death cases were caused by intracranial disease progressing,accounting for 26.03% of the total death toll,while the other 54 cases were caused by extracranial development of lung cancer,accounting for 73.97% of the total death toll.Overall survival rate was 39.17%,the average survival period was 13.295 ± 8.161 months.Half year survival rate was 71.6%.Tumor ease: CR in 16 cases(15.33%),PR 54 cases(45.00%),NC 30 cases(25.00%),PD 20 cases(16.67%).The overall treatment effective rate was 60.33%.In addition,there are 20 cases with tumor recurrence or new metastases in other areas in cranial choosing the gamma knife treatment.Single metastases,KPS > = 70 patients have longer life expectancy.Conclusion:Gamma knife treatment for brain metastases of lung cancer can effectively prolong patient survival,especially for patients with single brain metastases and KPS > = 70.And in the short term,the tumor can be effectively controlled.
