| Objective : To evaluate the clinical symptoms and the imaging changes of lumbar spine of lumbar disc herniation treated by percutaneous laser disc decompression(PLDD),To analyze the stability of lumbar spine and evaluate clinical effects of lumbar disc herniation treated by PLDD.Methods: 82 cases of patients with lumbar disc prolapse,in the changyi city people’s hospital of shandong province,was treated by PLDD form February 2013 to November2014.All the cases was divided into 3 groups according to the location of the disease,L4-L5 group,L5-S1 group,L4-S1 group(mixed group).The wavelength of 1064 nm ND-YAG pulsed laser line was used in the PLDD therapy.The clinical symptoms,the height of the intervertebral space,the height of the vertebral body and the height of the intervertebral foramen were observed at pre-operation,and 3,6,12,24 months after the operation.Also the displacement of the adjacent vertebral bodies in the lordotic kyphotic position was measured,the height index of the intervertebral disc was calculated,and the change of the volume of the intervertebral disc was evaluated.To study the effect on the stability of lumbar intervertebral disc protrusion treated by PLDD and to evaluate the clinical results.Results:A total 135 lumbar disc herniation of 82 cases(group L4-L5 13 cases;group L5-S1 16 cases;mixed group 53 cases)was treated by PLDD.The success rate of puncture was 100%.There were no significant difference between all stage in the height of the intervertebral space,the height of the vertebral body,the sagittal length of vertebral body,the index of intervertebral height and the height of the intervertebral foramen in all the groups(P > 0.05).There were significant difference in the intervertebral disc volume between the pre-operation and all postoperative period in all groups(P < 0.05).There is less than 3 mm in the displacement of adjacent vertebral body between the pre-operation and all postoperative period in all groups.There were no significant difference in the displacement of adjacent vertebral body and the curative effect between the pre-operation and all postoperative period in all groups(P > 0.05).Conclusion:There was obvious curative effect of lumbar intervertebral disc protrusion by PLDD,without compromise in spinal stability.PLDD Is a kind of effective minimally invasive treatment method. |
