The Analysis Of Intraoperative Neuromonitoring Of Recurrent Laryngeal Nerves In Endoscopic Thyroidectomy

Objective: The aim of this study was to compare the endoscopic thyroidectomy useing IONM(intraoperative neuromonitoring of recurrent laryngeal nerves)and the thyroidectomy without it.The laryngeal recurrent nerve injury is serious complications of the operation.It is said that the intraoperative neuromonitoring of recurrent laryngeal nerves has a lot of benefits for avoid the injury of laryngeal recurrent nerve.Compared with the two types of operation to find out the intraoperative neuromonitoring of recurrent laryngeal nerves is better or not in TET(total endoscopic thyroidectomy).Methods: The study include 74 patients undergoing totally endoscopic thyroidectomy at Qilu Hospital of,Shandong University in 2011 to 2014.There are 39 patients undergoing the TET with the IONM,and 35 patients undergoing the TET without it.Information about gender,age,operation data,intraoperative and postoperative complications,length of stay.Data were collected from patient medical records,survey,follow-up of outpatient and telephone.Results: No difference was noted between two groups in the term of demographic data for example gender,age,size of nodular diameter,proportion of benign and malignant.The group with IONM has shorter operating time than another(98.6 ± 37.9 min vs 136.7 ± 45.3 min,P<0.05).There is no difference between two groups in the intraoperative blood loss(23.3 ± 6.72 ml vs 26.5 ± 7.6 5ml,P>0.05).Postoperative pain was similar(2.3 ± 0.92 vs 2.0 ± 0.91,P>0.05).The two groups have similar Voice Handicap Index(VHI)(0.6 ± 0.31 vs 0.7 ± 0.42,P>0.05),The Reflux Symptom Index(RSI)(23.2 ± 7.21 vs 26.1 ± 6.80),postoperative bleeding(132.1 ± 40.50 ml vs 121.3 ± 30.65 ml P>0.05).Conclusion: The IONM has a lot of benefits on the smooth of the TET.It is easier to anatomic of laryngeal recurrent nerve.
