Objectives: The objective of this study is to explore the changes of MUC2,MUC5 AC,MUC6 and P53 expression in gastric cancer and preinvasive lesion and the relationship with clinical and pathological feature of early gastric cancer,to evaluation the feasibility in application of MUC2,MUC5 AC,MUC6 and P53 to identify tumor lesions in gastric mucosal biopsy specimens or non-neoplastic lesions in regular work,to definite whether the changes in expression of MUC2,MUC5 AC,MUC6 and P53 can provide a basis for more accurate diagnosis and treatment of gastric mucosa biopsy?Methods: A total of 125 specimens which included 30 cases of normal gastric mucosa biopsy specimens,38 cases of stomach high-grade intraepithelial neoplasia by ESD resection and 57 cases early gastric cancer by surgical specimens between Jan.1 2012 and Jan.1 2017 in Beijing Chao-Yang Hospital were entered in the study.HE and MUC2,MUC5 AC,MUC6,P53 immunohistochemical staining were performed for 125 sections.The interpretation standard of MUC2,MUC5 AC,MUC6 is: Positive cells number < 25% were negative(-);Positive cells number 25-50% for weakly positive(+);Positive cells number 50-75% for moderate positive(+ +);Positive cells number > 75% were strong positive(+ + +).The Interpretation standard of P53 is: Positive cells number > 10% notes for the P53(+),on the other hand,are recorded as P53(-).To compare whether there are some differences in the expression of MUC2,MUC5 AC,MUC6 and P53 among normal gastric mucosa,high-grade gastricintraepithelial neoplasia and early gastric cancer,as well as to compare whether there are some differences in the expression of MUC2,MUC5 AC,MUC6 and P53 in the and clinical pathological features,such as differentiation,infiltration depth,Lauren classification,mucus production,mucus production,lymph node metastasis and mesenchyme reaction in early gastric cancer.Results: 1.The positive rates of MUC2,MUC5 AC,MUC6 and P53 in normal gastric mucosa,intraepithelial neoplasia,and early gastric cancer.The positive rates of MUC2 in normal gastric mucosa,high-grade intraepithelial neoplasia and early gastric cancer were0.0%,22.6%,36.8%.The positive rates of MUC5 AC in normal gastric mucosa,high-grade intraepithelial neoplasia and early gastric cancer were 100%,50%,54.4%.The positive rates of MUC6 in normal gastric mucosa,high-grade intraepithelial neoplasia and early gastric cancer were 100%,60.5%,59.6%.The positive rates of P53 in normal gastric mucosa,high-grade intraepithelial neoplasia and early gastric cancer were 26.7%,68.4%,54.9%.The expression of MUC2,MUC5 AC,MUC6 and P53 in normal gastric mucosa,high-grade intraepithelial neoplasia and early gastric cancer were significant differences(P<0.05,the numbers are 0.000、0.000、0.000、0.003).The expression of MUC2,MUC5 AC and MUC6 in high-grade intraepithelial neoplasia and early gastric cancer did not have significantly differences.The expression pattern of MUC6 in high-grade intraepithelial neoplasia and early gastric cancer were changed,and there was no significantly difference between them.2.The relationship MUC2,MUC5 AC,MUC6,P53 in clinical pathological features of early gastric cancer.There were 9 cases in the intestinal type gastric cancer MUC2 was positive and the positive rate was 24.3%.While there were 12 cases in diffuse gastric cancer MUC2 was positive and the positive rate was61.9%.MUC2 negative and MUC2 positive of cases had significant difference in Lauren parting and positive correlation(P< 0.01,R=1).There were 9 cases in T1 agastric cancer P53 was positive and the positive rate was 36.0%.While there were 22 cases in T1 b gastric cancer P53 was positive and the positive rate was 68.8%.P53 negative and P53 positive of cases had significant difference in tumor infiltration depth and positive correlation(P < 0.01,R=1).Conclusion: 1.MUC2、MUC5AC、MUC6 may be an important method to distinguish cancerous and non-neoplastic lesions.2.P53 may be an important method to distinguish invasiveness and non-invasive lesions.
