Objectives Serum Cystatin C is found sensitive indexes of early kidney damage in recent years,it can not only reflect the elderly hypertension early kidney damage,and also can early prediction to the senile hypertension patients of cardiovascular events.Left ventricular hypertrophy is one of the most common cardiac damage in elderly hypertension and left ventricular mass index(bmi)is the criterion for diagnosis of left ventricular hypertrophy.Heart ankle vascular index is senile hypertension patients of aortic stiffness index of degree and vascular compliance.This study mainly discusses the senile hypertension patients serum(inhibition C and left ventricular mass index(bmi),heart ankle vascular index and clinic blood pressure,24-hour ambulatory blood pressure monitoring parameters,and discusses the dynamic pulse pressure index,the influence of the heart and blood vessels elastic damage to the kidney.Methods From April 2015 to April 2016 in North China Polytechnic University Affiliated Hospital of geriatrics Hospital of elderly patients with hypertension 122 cases,aged 60-86 years old,were accorded with the diagnostic criteria of hypertension guidelines for prevention and treatment of hypertension(2010 years)of the China version,for selected patients with 24 hours ambulatory blood pressure monitoring and recording ambulatory blood pressure the data.According to the APPI median of selected patients were divided into 2 groups: APPI = 0.41,A group,APPI<0.41 group B.The dynamic pulse pressure index was calculated according to 24 hour ambulatory blood pressure parameters: APPI=24 hour ambulatory pulse pressure,/24 hour mean systolic pressure.The basic data were collected and recorded for history and biochemical examination,laboratory,Cys C,at the same time,the patients with arteriosclerosis detector action calculated heart ankle vascular index,echocardiography,left ventricular mass index can be calculated according to Devereux formula.A clinical database was set up and analyzed by SPSS17.0 statistical software.The normal distribution of measurement data is expressed by means of mean + standard deviation.The difference analysis between the measurement data groups uses the t test,the rate or composition ratio indicates the distribution of count data,and the difference between groups is analyzed by chi square test.Logistic regression model was used to analyze the independent risk factors of related factors and blood pressure variability in the elderly.Spearman rank correlation analysis was used to study the correlation between variables.In the single factor correlation analysis,the correlation index(P<0.05)was used as the independent variable,and the multiple linear regression was used to analyze the influence degree of each factor.The difference of P<0.05 was statistically significant.Results 1 A group and B group in gender,age,body mass index(BMI),heart rate(HR)and the biochemical indexes of fasting blood glucose(FBG),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),systolic blood pressure(OSBP consulting room consulting room),diastolic blood pressure(ODBP)differences had no statistical significance(P>0.05).2,A,Cys,C,LVMI and CAVI were significantly higher than those in group B,the difference was statistically significant(P < 0.01).The relationship between the 3 Cys C levels and the dynamic pulse pressure index(APPI)was found.The lower the level of Cys and C,the lower the level of APPI,the higher the level of Cys and C,and the increase of APPI levels,and the two relations are quite close.The level of LVMI is related to APPI.The lower the level of LVMI,the lower the level of APPI,the higher the level of LVMI and the increase of APPI levels,and the two relations are very close.The level of CAVI is related to APPI.The lower the level of CAVI,the lower the level of APPI,the higher the level of CAVI and the increase of APPI levels,and the two relations are closely related.Multivariate L ogistic regression analysis showed that Cys,C,LVMI and CAVI were independent risk factors of ambulatory pulse pressure index(beta >0,OR>1).4,24 h SBP,24 h DBP,d SBP and n SBP are related to Cys and C,among which 24 h SBP and 24 h DBP have the largest correlation with Cys and C,while OSBP and ODBP have nothing to do with Cys and C.Age,24 h SBP,24 h DBP,d SBP and n SBP were related to LVMI,among which 24 h SBP,24 h DBP and LVMI had the greatest correlation,while OSBP and ODBP had no correlation with LVMI.24 h SBP,24 h DBP,d SBP,n SBP and n DBP are related to CAVI,in which 24 h SBP,24 h DBP and CAVI have the largest correlation,while OSBP,ODBP and CAVI have no correlation.Conclusions 1 In elderly patients with hypertension,with the increase of dynamic pulse pressure index,serum cystatin C,left ventricular mass index,heart and ankle vascular index were increased.2 In elderly patients with hypertension,serum cystatin C,left ventricular mass index,cardio ankle vascular index are independent risk factors of elevated pulse pressure index.3 In elderly hypertensive patients,the average systolic pressure of 24 hours and the average diastolic pressure of 24 hours have the greatest influence on Cys C,LVMI and CAVI.
