Objective:To detect the serum anti PLA2 R antibody(PLA2R-Abs)and Helicobacter pylori infection in patients with idiopathic membranous nephropathy,and to explore the relationship between Helicobacter pylori infection and serum anti PLA2 R antibody and clinical parameters in patients with idiopathic membranous nephropathy.Methods:1 A total of 60 patients with idiopathic membranous nephropathy were enrolled in the Department of Kidney Medicine,Xingtai People’s Hospital from January 2014 to June 2016.The general information of the patients was recorded.2 Experimental group1)according to the degree of proteinuria in patients with IMN,the patients were divided into mild risk(proteinuria < 4.0g/d)group,medium risk(urinary protein 4-6g/d)and high risk(urine protein > 6.0g/d)group.2)According to the results of serum antibody detection,they were divided into PLA2R-Abs positive group and PLA2R-Abs negative group.3)According to the results of 14 C breath test,HP infection group and HP infection group were divided into two groups.3 The detection of serum PLA2R-Abs was performed by double-antibody two-step sandwich enzyme-linked immunosorbent assay(ELISA).The infection of Helicobacter pylori was detected by 14 C breath test.4 Statistical analysis: using SPSS software for statistical data processing with Microsoft Excel database.Results:1 The positive rate of serum PLA2R-Abs in 48 cases,the positive rate was 80%.IMN patients with PLA2R-Abs positive serum albumin was significantly lower than PLA2R-Abs negative group,the difference was statistically significant(P<0.05),urine protein,cholesterol,creatinine was significantly higher than that of negative group increased,the difference was statistically significant(P<0.05).2 There were 48 cases of type M serum PLA2 antibody greater than 0.1ug/ml,the positive rate of 80%.The serum PLA2R-Abs concentration in the high risk group was significantly higher than that in the control group and the mild group(P<0.05).Serum albumin increased with the degree of proteinuria was decreased(P<0.05);blood cholesterol and glycerin three fat levels gradually increased with the increase of the degree of proteinuria(P<0.05);a high-risk group of patients serum creatinine was significantly higher than that in the moderate group(P<0.05).3 HP positive results in 43 cases,the total infection rate was 71.7%.Compared with the negative group,the levels of serum creatinine and urine protein were significantly increased in the HP positive group and the negative group,the difference was statistically significant(P <0.05);positive group serum albumin was significantly lower than the negative group,the difference was statistically significant(P<0.05).4 The positive rate of serum PLA2 R was significantly higher in HP positive group than that in HP negative group,and the level of serum PLA2 R was significantly higher than that of HP negative group(P <0.05).5 The linear correlation analysis between PLA2R-Abs and each variable showed that the anti PLA2 antibody was significantly positively correlated with urinary protein quantitation and serum creatinine and negatively correlated with serum albumin(P<0.05).Linear analysis of HP infection and various variables showed that HP infection was positively correlated with urinary protein quantitation and serum creatinine,and negatively correlated with serum albumin(P<0.05).Conclusion:1 Serum PLA2R-Abs is a noninvasive specific biological marker for the diagnosis of IMN,and is closely related to IMN.2 IMN patients often accompanied by HP infection,HP infection and serum PLA2R-Abs and urinary protein,serum albumin,serum creatinine correlation. |
