| Background: Chronic Heart Failure(CHF)is a common end-stage performance of cardiovascular disease.The CHF is the end-result for most cardiovascular diseases.Because of the characteristics of repeated,CHF has a serious influence on the patient's quality of life,and causes a huge economic burden for their family and society.Epidemiological studies found that the morbidity and mortality of CHF increased year by year,and became one of the two most challenge major cardiovascular diseases in 21 st century.Western medicine works fast in treating heart failure,but due to the side effects,the expensive price,and together with the progression of chronic heart failure is long,make the clinical application of such western medicine restricted.Combination of traditional Chinese and western in prevention and treatment of CHF has been widely recognized by clinical workers because of its stable curative effect and high safety.According to the clinical experience of many years and the theory of Chinese medicine and Chinese-western combined medicine,Professor Miao Huawei invented the Nourishing Qi Activating Blood Circulation and Diuresis prescription in treating the CHF,and achieved a satisfactory result.Objective: To observe the clinical effect of the Nourishing Q Activating Blood Circulation and Diuresis prescription invented by Professor Miao in the treatment of CHF and to evaluate its safety as well as efficacy.Methods: This study strictly abided the diagnosis and inclusion criteria,70 patients with CHF(Qi deficiency and blood stasis edema type)and cardiac insufficiency in level ?,or according to NYHA were collected from ? ?Hebei Province Chinese Medicine Hospital outpatient.They were divided into two groups randomly: 35 patients in treatment group and 35 in control group.All included patients were required to proper rest with limited water and sodium intake.The patients of control group were treated with conventional western medicine,including cefuroxime Sammy 20 mg,bid;spironolactone 20 mg,qd;benazepril 10 mg,qd;tartrate metoprolol 12.5mg,bid.We formulated individual treatment plans according to the state of illness.Patients in treatment group were treated with the same routine western medicine treatment combined with the Nourishing Qi Activating Blood Circulation and Diuresis prescription,a dose daily,take orally two times one day at morning and night.Each group was treated for 4 weeks.Observed and recorded each index of patients in two groups before and after medication,including: clinical symptom,physical sign,blood pressure,heart rate,rhythm of the heart,heart function classification,6 Minutes Walk Test(6MWT),Left ventricular ejection fractio(LVEF),N-terminal pro-brain natriuretic peptide(NT-proBNP),blood routine,urine routine,liver function,kidney function,electrolyte,etc.Results:1 After 4 weeks treatment and observation,the total effective rate was 91.39% in the treatment group(Of those 35 patiens,12 cases had marked effect,20 cases effective,3 had no effect);the total effective rate was 71.43% in the control group(Of those 35 patiens,10 cases were markedly effective,15 cases effective,10 cases were invalid).The treatment group is significantly higher than the control group in total effective rate(P<0.05).2 There were significant improvements in 6MWT in the same group(P<0.05).The improvement of treatment group was higher than the control group,there was statistical difference between two groups(P<0.05).3 The NT-proBNP levels of patients in both two groups were significant decreasing after treatment(P<0.05).The treatment group was superior to the control group,there was statistical difference between two groups(P<0.05).4 There were significant improvements in LVEF in both groups(P<0.05),and the improvement of treatment group was higher than the control group,there was statistical difference between two groups(P<0.05).5 There were no abnormal changes between two groups of patients regarding with the blood and urine routine,liver and kidney function,electrolyte and the signs of life,before and after treatment.Conclusions:Chinese medicine Nourishing Qi Activating Blood Circulation and Diuresis prescription is an effective prescription in treating CHF.It has definited curative effect on CHF(Qi deficiency and blood stasis edema type),it could relieve the symptoms,improve the cardiac function,prolong the patient's distance of 6MWT and decrease the NT-proBNP safely without side effects.This study provides a new thought of treatment of CHF and improve the quality of life of those patients,delay the process of heart failure,also provided a objective basis for combining traditional Chinese and western medicine treatment of CHF. |
