| Objective To explore the diagnosis value of the multiple parameters in Diffusion Kurtosis Imaging about differenting high-grade glioma and solitary brain metastases.Methods 19 patients of HGG and 14 patients of SBM confirmed by pathology or clinical follow-up have been collected.All patients underwent the examination of MRI conventional scanning,DKI scanning and enhanced scanning before treatment.ROI were drawn to record the DKI parameters,which were corrected to get the magnitudes of FA,MD and MK.Compare the values between the two groups by independent samples t test and draw ROC curves with statistically significant parameters.Results The FA,MD and MK average values in the solid area and surrounding region of HGG were 0.432±0.130,1.465±0.402,0.652±0.188;0.176±0.047,1.766±0.326,0.565±0.083.And the FA,MD and MK average values of above-mentioned regions of SBM were 0.383±0.120,1.647±0.332,0.717±0.176;0.142±0.037,2.024±0.221,0.460±0.057.FA,MD and MK value of the solid region were not statistically significant in the differences between the two groups.FA,MD and MK value of the surrounding region were statistically significant in the differences between the two groups(P<0.05).ROC analysis showed that when the peritumoral FA was 0.161,the sensitivity and specificity for the differentiation of the two tumors was 68.4% and 71.4% respectively,the AUC was 0.722;When the peritumoral MD was 1.910,the sensitivity and specificity for the differentiation of the two tumors was 73.7% and 71.4% respectively,the AUC was 0.756;When the peritumoral MK was 0.516,the sensitivity and specificity for the differentiation of the two tumors was 84.2% and 85.7% respectively,the AUC was 0.865.The sensitivity and specificity of peritumoral MK value is higher than those of peritumoral FA and MD values in differentiating the two tumors.And the AUC for peritumoral MK value showed the largest.Conclusions The peritumoral FA,MD and MK value could differentiate high-grade glioma from solitary brain metastases and the peritumoral MK metric diagnostic efficacy was the highest. |
