| Objective: By immunohistochemical staining and HE staining,oil red O staining and Masson staining method,in human carotid atherosclerotic plaque samples,within the plaques on the lipid composition of composition,macrophages,smooth muscle composition and composition of collagen fiber staining,respectively,at the same time,using the method of counting plaques microvascular density and determination of plaque within 7 types of Epidermal Growth Factor structure domain(Epidermal Growth Factor like domain7,EGFL7)protein expression in human carotid atherosclerotic plaques,explore EGFL7 expression level and plaque formation conditions,the relationship between capillary and EGFL7 expression levels and microvascular generation and between the relationship between carotid atherosclerosis plaque stability.Methods: The atherosclerotic plaques were obtained from patients who undergo carotid endarterectomy surgery performed by experienced neurosurgeon in the Department of Neurosurgery,Affiliated Hospital of Qingdao University,from June 2015 to July 2016.All plaques were collected and divided into 36 groups.Preoperative for all patients with bilateral neck vascular ultrasound,head,neck,and computer tomography angiography(CTA)and transcranial doppler(TCD),of which one for elderly women with obvious symptoms of myocardial ischemia to be ruled out.For performance are clear bilateral internal carotid artery stenosis(stenosis rate > 50%)of patients,preference of symptoms such as a transient ischemic attack(TIA)or stroke occurs on one side of the vascular blood vessels(responsibility)for surgical treatment.According to the histopathologic characteristics,grouping situation for vulnerable plaques group number for 15 cases(7 male and 8 female),stable plaque group number 20 cases(10 male and 10 female).Labeling was performed on formalin-fixed,paraffin-embedded,4μm tissue sections on polylysine-coated–plus glass slides.According to the diagnosis of the HE-stained specimen by two experienced histology doctors who depend on the plaque classification of AHA(American Heart Association),the plaque tissues were then divided into two groups which were stable-plaque-group and vulnerable-plaque-group.The expression of EGFL7 and CD34 CD68 α-SMC in carotid atherosclerosis was observed in detecting the average optical density by means of immunohistochemistry technique,and the development of microvessels was determined by the microvessel density(MVD).Results: To our knowledge,the vulnerability index of stable plaque group and vulnerable plaque group are repectively(0.40±0.16)(1.97±0.63),the difference was statistically significant(t=9.448,P < 0.05).In stable plaque group EGFL7-positive cell area were(0.093±0.05),higher than the area of positive cells in vulnerable plaques which were(0.213±0.41),the difference was statistically significant(t= 7.805,P < 0.05).The MVD in stable plaque group(30.500±18.03),lower than the vulnerable plaque group(127.867 ±40.79)the difference was statistically significant(t= 8.634,P < 0.05).The MVD count and plaque index were positively correlated(r= 0.690,P < 0.05).EGFL7 positive cells and MVD count were positively correlated(r=0.690,P < 0.05).EGFL7 positive cells and plaque vulnerability index showed positive correlation(r=0.873,P < 0.05).Conclusion: 1.EGFL7 high expression in endothelial cells,promotes endothelial cell function,promote blood vessel formation.2.CD34 expression level is high,the MVD value high patches often accompanied by increased vulnerability coefficient,plaque stability.3.EGFL7 high expressed in vulnerable plaques,and there is a positive correlation relationship and vulnerability coefficient,prompt patchs EGFL7 promote angiogenesis function reduces plaque stability. |
