A Clinical Study Of The Effects Of Low Lever Laser Therapy On The Orthodontic Gingivitis

Objective :With the improvement of people’s living conditions,people’s aesthetic requirements are increasing.Therefore,more and more patients choose the fixed orthodontic treatment to correct malocclusions.Gingivitis is one of the common complications for patients with fixed orthodontic treatment,oral health education and routine periodontal treatment are usually not perfect.Studies have shown that Low level laser(LLL)play an important role in biological regulation,it can reduce the inflammation of issue and promote the wound healing.In this study,through clinical trials,to explore the effects of low level laser(LLL)in gingivitis with fixed orthodontic treatment,provide reference of fixed orthodontic gingivitis to the oral clinical workers,and provide a theoretical basis for the application of LLL in orthodontic clinic.Methods:20 patients(12 males,8 females)who accepted fixed orthodontic treatments and suffered from gingivitis meanwhile were chosen.They are 12~18 years old,with an average age of 15.2 years.All the subjects received regular oral hygiene instruction and periodontal scaling before the experiment,removing the calculus and plaque thoroughly.After one week of the periodontal treatment,each patient choose 31,32,41,42 four teeth of each patient were chosen as the index teeth and the left and right side were randomly divided into experimental group and control group respectively.Four periodontal clinical indicators of the plaque index(PLI),gingival index(GI),bleeding index(BI)and probing depth(PD)and the concentration of IL-1 beta in gingival crevicular fluid in two groups were recorded as the " before treatment".The experimental group was treated with low level laser therapy(LLLT)of 650 nm and 25 m W,1 time / day for 3 days and the control group received no treatment.One week later,the four clinical parameters and the concentration of IL-1 beta in gingival crevicular fluid of the experimental group and control group were recorded again,the statastics were set as ”after treatment”.All examinations should be carried out by the same physician.The correlated index between treatment group and treatment-ended group was analysed by statistical method.Results: 1.Periodontal indexes: The difference of PLI、GI、BI、PD in experimental group and control group before treatment were not statistically significant,but the differences were statistically significant after treatment,and the results of the four indicators were significantly reduced in the experimental group.the differences were statistically significant(p<0.05).2.The concentration of IL-1 beta in gingival crevicular fluid: The difference of IL-1 beta concentration in experimental group and control group before treatment was not statistically significant,but the difference was statistically significant after treatment,and the concentration of IL-1 beta was significantly reduced in the experimental group(p<0.05).Conclusion: LLL has a preventive and therapeutic effect on gingivitis during fixed appliance therapy.
