Objective To investigate the postoperative pain status among patients with adenomyosis undergoing ultrasound ablation,and explore the effectiveness of music therapy,which will finally provides the reference for clinical pain management.Methods 1 A questionnaire survey was conducted at the First Affiliated Hospital of Chongqing Medical University on February 2015 in patients with adenomyosis undergoing ultrasound ablation.The investigators used the self-designed "patient information registration form" 、 "Houston pain questionnaire" and "Visual analogue scale" to understand the patient’s general condition and postoperative pain.2 The subjects who had severe pain within 2 hours after the operation were interviewed as target interviews and semi structured interviews were conducted at the time of discharge from the hospital with the theme "postoperative pain perception".Understand the patient’s pain experience and psychological status 3 Convenience sampling was adopted and a total of 100 patients with adenomyosis were selected from HIFU Tumor Therapeutic Research Center of The First Affiliated Hospital of Chongqing Medical University,they were randomly divided into the control group and the trial group:control group only received routine pain management and the trial group increased music therapy on the basis of it.Intervention time from the moment into the operating room to 2 hours after surgery.Results 1.The postoperative pain A total of 55 cases of adenomyosis patients,the postoperative 2h pain incidence rate reached as high as 98.2%,there were 46 cases with moderate or more pain,accounting for 83.6%;a total of 44 cases of abdominal pain,the incidence was 81.5%;9 cases of sacroiliac and hip pain,with a rate of 16.7%;1 cases of lower limb pain,with a rate of was 1.8%.The postoperative analgesic utilization rate was 58.2%,and the postoperative 24 h gradually became mild pain.2.The effect of postoperative pain on patients As follows: in terms of emotion,the main performance was “don’t want to do anything”(7.48±1.59),“have to rely on others”(7.28±1.47),,“scared or nervous”(7.19±1.50)”,“worry about pain means something”(7.00±1.79);The most unsatisfactory aspects about postoperative pain management for patients were “pain relief”(5.31±1.13),“pain care”(5.74±1.18),“encourage family members help to control pain”(6.00±1.83);for them,the most unsatisfactory aspects about pain control education were “non-pharmaceutical pain control method”(0.67±1.30),“the side effects of pain medication”(1.49±2.39),“painkillers to control pain”(1.89±2.47).3.The intervention effect of music therapy 3.1 Two groups of patients with intraoperative heart rate changes: When patients on the operating table,the heart rate of the control group and the trial group was 93.34±17.56 and 87.60±17.18,the difference was not statistically significant(P>0.05).When the operation began after 0.5h,1h,and at the end of the surgery,patients in trial group had lower heart rate(94.02±12.84 vs 100.48±14.82 、 91.74±12.97 vs 96.50±14.47 、88.76±10.96 vs 94.58±15.38,P<0.05)than patients in the control group.3.2 Comparison of postoperative pain between the two groups:at 0.5h after operation,the pain between the trial group and the control group was not statistically significant(P>0.05);at 2h after operation,patients of the trial group had lower pain than those in the control group(4.06±1.88 vs 5.86±1.08,P<0.05).3.3 When compared with the control group,the number of use painkillers was less than those in the trial group(12 vs 28,P<0.05).Patients in the trial group were more satisfied with postoperative pain management(8.38±1.41 vs 5.20±0.94,P<0.05)than those in the control group.Conclusion 1 The incidence of pain in patients with adenomyosis after ultrasound ablation is high,especially in 2 hours after operation.The degree of satisfaction with pain management is low.Medical staff should attach great importance and take active intervention measures.2 Music therapy can effectively alleviate the pain of adenomyosis after ultrasound ablation and improve the satisfaction of pain management.
