Objective Through the results of the baseline survey to understand the health status of cardiovascular disease in rural areas over 50 years old in rural areas of Wulong County,Chongqing,Predict the cardiovascular disease risk rating of the subjects through the WHO / ISH risk prediction figure. To evaluate the effect of grading intervention on the intervention of different risk groups, and to provide suggestions for promoting the classification of cardiovascular disease risk groups.Method Subjects were randomly selected from the rural areas of Wulong County,Chongqing, where they were randomly selected from the local towns.Subjects included in the standard:(1)age ?50 years, with a certain understanding;(2)hypertension,diabetes or dyslipidemia patients or smokers;(3)agreed to participate in the study.Subject to the exclusion criteria:(1)previous cognitive, mental retardation;(2)suffering from fatal or non-fatal cardiovascular and cerebrovascular diseases;(3)previous history of mental illness. The questionnaire was used to develop the questionnaire, which included the basic information of the subjects, the smoking status, the diet, the physical activity, the high blood pressure, the presence of diabetes, the presence of diabetes, whether it was suffering from diabetes, whether it was suffering from diabetes, whether it was suffering from diabetes, whether it was suffering from diabetes dyslipidemia, cardiovascular disease warning symptoms and risk factors knowledge, the study of the height, weight, systolic blood pressure, cholesterol and other physiological and biochemical indicators.Baseline surveys were conducted on the subject, according to gender, age, systolic blood pressure classification, total cholesterol score, smoking and whether to have diabetes, using the World Health Organization 2008 Cardiovascular Risk Assessment and Management Pocket Guide in WHO / ISH risk prediction figure to predict the risk of fatal or nonfatal cardiovascular events over a decade,at the same time, according to the risk level of the research object,it is divided into three groups: low risk group(predict 10 years risk level<10%),middle risk group(predict 10 years risk level<20%)and high risk group(predict 10 years risk level?20%).Risk population classification intervention team consists of clinicians, public health physicians, nurses and other components, and community-based study of the object for 6 months of grading intervention. To evaluate the effect of risk factors for risk assessment, the smoking effect, the diet, the improvement of the diet, the knowledge of the cardiovascular disease and the changes of the physiological and biochemical indexes were evaluated by the risk stratification of the subjects before and after the grading intervention. The median(lower quartile, upper quartile)(M(P25,P75))is used to describe the metrological data, the rate and composition ratio are used to describe the count data;using the rank sum test of the paired data,Chi-square test analysis of grading intervention before and after the changes in the indicators. Results 601 people volunteered to participate in the study,of which 3 were lost in the course of the study(loss rate 0.50%),a total of 598 people completed the entire classification intervention study process.Baseline survey shows,328 females(54.85%),male 270(45.15%),male to female sex ratio 0.82: 1;average age(70.7±7.15)years;education level to not receive formal school education(44.15%)and primary school did not graduate(24.58%);Hypertensive patients in 440 cases(73.58%),diabetes in 56 cases(9.36%),dyslipidemia or hyperlipidemia in 14 cases(2.34%);168 cases(28.09%)were smokers,146 cases(24.41%)were drinkers,283 cases(47.32%)were overweight(BMI ? 24kg/m2).There were 235(39.30%)in the low risk group,209 in the middle risk group(34.95%)and 154(25.75%)in the high risk group. Before and after the intervention, the comparison of the risk predictio n grade of cardiovascular disease showed that the difference was statistically significant(P <0.05).Before and after the intervention, the results of the comparison of smoking, lifestyle, cardiovascular knowledge, physiological and biochemical indexes, etc.,before and after the intervention, showed whether the subjects had daily smoking, the frequency of exercise, the time of each exercise, the amount of each exercise is more than 6000 steps, readme whether to take salt reduction measures, the intake of vegetable oil?salt intake?intake of red meat?intake of fresh fruit(g/d),there were significant differences in the symptoms of stroke, the difference of symptoms, body mass index(kg/m2),systolic blood pressure(mm Hg),total cholesterol(mmol/L),high density lipoprotein cholesterol(mmol/L)and fasting blood glucose(mmol/L)(P<0.05);The intake of animal oil intake?poultry intake?intake of aquatic products?fresh vegetable intake(g/d),cardiovascular and cerebrovascular disease risk factors,triglyceride(mmol/L)and low density lipoprotein cholesterol(mmol/L)were not statistically significant(P>0.05).Conclusion Diet and exercise guidance, smoking cessation and limited alcohol guidance, compliance with medical guidance(antihypertensive drugs, hypoglycemic drugs and the use of lipid-lowering drugs)and cardiovascular disease-related knowledge and education and other hierarchical interventions to improve the Wulong County,Chongqing, risk predictive level of population at risk for cardiovascular disease in the region, specifically to a certain extent,to improve their lifestyle, health and cardiovascular disease-related knowledge of the situation. But should also increase the intervention time, increase the intensity of intervention, especially in the middle-risk and high-risk group of interventions, rich levels of interventions to help the respondents to improve the awareness of smoking cessation, reasonable dietary awareness and behavior improvement to reduce the risk level of cardiovascular disease risk population, improve their health and quality of life.
