Clinical Features Of Non-compaction Cardiomyopathy In Children And Model Construction | | Posted on:2018-03-16 | Degree:Master | Type:Thesis | | Country:China | Candidate:H M Zhou | Full Text:PDF | | GTID:2334330536472354 | Subject:Academy of Pediatrics | | Abstract/Summary: | PDF Full Text Request | | Part 1 Clinical Characteristics in Children with Non-compaction CardiomyopathyObjective Criteria of non-compaction cardiomyopathy(NCC)by ultrasonography in children is not yet entirely clear.Nearly 90.1% published papers regard N/C>2.0 as the diagnostic criteria,while others approve N/C>1.4.Comparative Analysis of clinical features and prognosis between the two groups(N/C>2.0 and N/C∈(1.4,2.0])is needed to form a deeper insight into NCC in children for the clinician.Methods Patients with NCC were divided into two groups according to the ratio of non-compacted myocardium to compact myocardium at the end of systole(N/C),Group N/C>2.0 and Group N/C ∈(1.4,2.0].Retrospective control study was used to analyze the clinical features of the two groups and phone call interview was used to learn their prognosis.Results There are no significant differences in clinical features including ordinary conditions,heart damage and complications between the two groups.But Group N/C>2.0 have a lower recovery rate(8/26 vs 18/26,p=0.000)and a higher mortality(p=0.008)than Group N/C∈(1.4,2.0] three years after diagnosis as NCC for the first time.Conclusions Compacted to Group N/C∈(1.4,2.0],Group N/C>2.0 showed more deteriorate outcomes.Part 2 Alcohol Exposure during Pregnancy cause the Similar Change of Non-Compaction CardiomyopathyObjective Non-compaction Cardiomyopathy(NCC)is a type of inherited cardiomyopathy with unknown pathogenesis.Most of cases are sporadic.But there aren’t any published papers about poison exposure during pregnancy caused NCC.After a clinical follow-up visit to NCC patients,we found that high-dose alcohol exposure may be a potential pathogenesis in the formation of NCC during early pregnancy.Methods Pregnant mice were given 56% alcohol with the dose of 5ml/kg during ED3.5-ED18.5 by gavage.Transmission electron microscopy and HE staining were used to verify the structure change of myocardium for ED19.5 mice.Echocardiography was used to evaluate the cardiac function of NCC patients after growing up.Results High-dose alcohol exposure during pregnancy cause the disorganized and dissolved change of myofilament,and the similar change of non-compaction cardiomyopathy in the alcohol exposure group.Heart function failed and myocardium was thickened in offspring mice after they grow up.Conclusions High-dose alcohol exposure during pregnancy cause the similar change of NCC,and it may be a potential pathogenesis of NCC. | | Keywords/Search Tags: | Non-compaction cardiomyopathy, Children, Diagnostic Criteria, Clinical Features, Prognosis, Pregnancy, Alcohol Exposure, Offspring, Non-Compaction Cardiomyopathy | PDF Full Text Request | Related items |
