Clinical Characteristics And Analysis Of Hashimoto’s Thyroiditis Combined Papillary Thyroid Carcinoma

Objective:Nowadays more and more cases of Hashimoto’s Thyroiditis be complicated by papillary thyroid carcinoma("PTC")are found.To study these clinical features,we comparatively analyze with simple PTC,thus enhance the acknowledgement to this disease,to support to diagnosis/treatment/prognosis evaluation.Methods:Retrospectively study cases given by Shanxi Medical University affiliated Shanxi Province Tumor Hospital,from Nov.2015 to Nov.2016,57 cases which after head and neck surgery,clinicopathologically analyzed Hashimoto’s Thyroiditis be complicated by PTC,named as A group;Those simple PTC,total 116 cases,named as B group.Compare gender,age,tumor diameter,Preoperative TSH value,microcarcinoma incidence rate,the central group of lymph nodes transfer rate,TNM stage Feature.Results:1.In A group,50 female patients,87.7% of total;In B group,76 female patients,65.5% of total,P<0.000,the difference has statistical significance;2.Age of A group,42.3+/-13.6,while age of B group,45.7+/-14.5,P=0.142,the difference has no statistic significance;3.Tumor diameter,A group,1.46+/-0.87 cm,B group,2.02+/-1.21 cm,P < 0.000,difference has statistic significance;4.Preoperative TSH Value,A group,2.34+/-2.12 m IU/L,B group,1.73+/-1.56 m IU/L, P=0.028,the difference has statistic significance;5.Microcarcinoma Incidence Rate: A group,42.1%,B group,37.1%,P=0.523,the difference has no statistic significance;6.The central group of lymph nodes transfer rate,A group,24.6%,B group,45.7%,P=0.007,the difference has statistic difference;7.TNM stage Feature: A group,stage one,43 patients,75.4% of total,stage two,3patients,5.3%,stage three,6 patients,10.5%,stage four,5 patients,8.8%;B group,stage one,35 patients,30.2%,stage two,24 patients,20.7%,stage three,39 patients,33.6%,stage four,18 patients,15.5%.P<0.000,the difference has statistic significance.Conclusion:Hashimoto’s Thyroiditis be complicated by PTC,the female patients are diagnosed more than male ones,their preoperative TSH values are apparently higher than those of simple PTC patients,this figure can help distinguish the two diseases,and coincident shorter tumor diameter,lower transfer rate of the central group of lymph nodes,earlier clinical stage,all of the above indicate that Hashimoto’s Thyroiditis may impede the aggravation of PTC.
