| ObjectivesTo explore the clinicical value of Japan Esophageal Society classification and endoscopic ultrasonography in the assessment of invasion depth of early esophageal carcinoma.MethodsData of 98 cases of early esophageal carcinoma diagnosed from January 2016 to December 2016 were studied retrospectively,Intraepithelial papillary capillary loop(IPCL)of all lesions were classified based on AB claafication,the invasion depth were evalauted by endoscopic ultrasonography,and were compared with pathologic examination performed after endoscopic submucosal(ESD)or surgery to evaluate the accuracy,sensitivity,specificity,and compared AB classfication with endoscopic ultrasonography.ResultsOf all the 86 leisions’ invasion depth,according to the assessment by endoscopy before operative,49 cases of B1,31 cases of B2,6 cases of B3,the accuracy of AB classfication is 77.91%.In type B1,the accuracy,sensitivity,specificity of M1 and M2 was 83.67%,85.42%,78.95%;In type B2,the accuracy,sensitivity,specificity of M3 and SM1 was74.19%,85.19%,86.44%;In type B3,the accuracy,sensitivity,specificity of SM2 was50%,75%,96.34%.according to the assessment by endoscopy before operative,68 of mucousal layer,17 of submucosal layer,the accuracy of endoscopic ultrasonography is 86.05%,in mucousal layer,the accuracy,sensitivity,specificity is 89.71%,93.85%,66.67%;in submucousal layer,the accuracy,sensitivity,specificity is 76.47%,92.86%,94.44%;there was no statistically significance difference between AB classfication and endoscopic ultrasonography.ConclusionsAB classfication is with certainly accuracy,but some of the sub-groups are limited in the accuracy.Endoscopic ultrasonography is with certainly accuracy,too.AB classfication and endoscopic ultrasonography are valuable for the diagnosis of invasion depth of early esophageal carcinoma,there was no statistically significance difference between them,and present suggested that various methods should be combined on the assessment of invasion depth of early esophageal carcinoma,especially for doubt cases. |
