Objective The purpose of this study was to evaluate the effectiveness and risk factor of bipolar radiofrequency maze operation for atrial fibrillation(AF)during valvular replacement procedure.Methods From January 2012 to May 2014,128 patients underwent bipolar radiofrequency maze operation for AF during cardiac surgical procedures.The postoperative indexes of electro-cardiogram,echocardiogram and NYHA class compared through retrospective analysis and follow-up.By comparing grouping preoperative and postoperative related information,statistical analysis are conducted to calculate the influence of risk factors for postoperative recurrence.Results 128 patients were alive in the early postoperative period,without high conduction block.The stable sinus rhythm(s SR)in postoperative 6 months and 12 months or more were 84.38% and 85.93% respectively.Echocardiography showed significant reduction compared with preoperation in left atrial diameter,ejection improved significantly.Single factor analysis showed that the differences of left atrial diameter,AF duration and moderate-severe tricuspid regurgitation are of statistical significance and are the influencing factors of surgical effect.Multivariate logistic regression analysis showed that preoperative left atrium diameter,the duration of atrial fibrillation and moderate-severe tricuspid regurgitation(left atrium diameter greater than 60 mm)are risk factors affecting the effects of surgery.Conclusion Bipolar radiofrequecy maze operation is an effective surgical option for treating AF.The three-month postoperative cardioversion rate of sinus rhythm increase gradually,AF recurrence rates decreased.The AF course time of more than60 months,the left atrium diameter > 60 mm and moderate-severe tricuspid regurgitation are the independent risk factors influencing the bipolar radiofrequency ablation curative effect.
