Objective:To study the Difference in blood lipid and urinary protein and factors influencing blood pressure with different types of hypertension.Methods:The inritial treatment children with primary nephrotic syndrome(n=101)were chosen from the inpatients of our hostipal from March.2014 to March.2016.They were named treatment group.Meanw hile,61 patients hospitalized in our hospital with upper respiratory infection(URI),whose group is named matched group.None of them were taking anti-hypertensive drugs before this test.The indexes of medical history,height,weight,blood pressure,were recorded and detected.The biochemical indexes were detected by using Cobas-8000 automatic chemiluminescence analyzer,including cholesterol(CHO),triglyceride(TG),high-density lipoprotein-cholesterol(HDL-C),low-density lipo-proteincholesterol(LDL-C),apolipoproteinA(ApoA),apolipoprotein B(ApoB),lipoprotein(a).All cases were divided,acc ording to blood pressure values,into normal pressure group(n=21),isolated systolic hypertension(ISH group,n=18),isolated diastolic hypertension group(IDH group,n=18)and systolic-diastolic hypertension group(SDH group,n=44).The diffrence of detected indexes between treatment and matched groups were been made by using independent sample t-test.All the detected indexes were compared and studied among 4 groups and observed by using one way-ANOVA and nonparametric tests.And the factors influencing blood pressure were made by using regression analysis.Results:The results showed that CHO,TG,HDL,ApoA,Lp(a),systolic bloodpressure(SBP)and diastolic blood pressure(DBP)were significantly higher in treatment group than matched group(P<0.05).The difference of LDL and ApoB by using nonparametrc test analysis,showed that obvious difference in the treatment group and matched group(P<0.05).And the children with PNS can be divided into four groups,according to the blood pressure.Using the one way analysis of variance,we could found the results in each group with onset gender and age structure has no obvious difference(P>0.05).The levels of CHO,TG,and LDL-C were significantly higher in SDH group,than those three groups(P<0.05).The level of ApoA was significantly higher in ISH group than those three groups(P<0.05).Four groups of comparisons,the levels of Lp(a),HDL-C,ApoB were found no obvious difference(P>0.05).The 24 hours urine protein of the SDH group was obviously higher than that of the rest of the three groups(P<0.05).The level of 24 hours urinary protein in ISH group was higher than normal blood pressure group(P<0.05).And the difference of the level of 24 hours urinary protein between IDH group and nomal pressure group was no obviously(P>0.05).The level of LDL-C was higher in SDH group than the rest of the three groups(P<0.05),and among the rest of three groups,there were no significant diffrence(P>0.05).The results of correlation analysis showed that CHO,TG,ApoA and 24 hours urine protein were positively correlated to SBP,and CHO,TG,and 24 hours urine protein were positively correlated to DBP.Conclusion:According to the characteristics of the different types of blood pressure and the metabolism disorder of blood lipid,there is more indispensable to monitor the blood pressure dynamically in the clincal trails.The classification diagnosis of the blood pressure of PNS children,and the detailed of diagnostic standard of the classifaction of high blood pressure will be more improtant to enhance the clinic level in PNS.And to found the theory basic of therapeutic method of PNS.
