| Objective: At present,sarcomatoid lung cancer is a relatively rare malignant tumor.In recent years,the incidence of lung sarcomatoid carcinoma has been increasing,which has aroused widespread concern of thoracic surgeons,and its clinical features have become a hotspot in recent years.This study was to investigate the clinical characteristics of lung sarcomatoid carcinoma and explore the relationship between clinical features and prognosis.Methods: Fifty patients with lung sarcomatoid carcinoma were enrolled in the Fourth Hospital of Hebei Medical University,including 40 males and 10 females.Patients aged between 24 and 75 years,median age of 62 years.Patients with more than chest,back pain,cough,sputum,sputum bloody or hemoptysis for the initial symptoms to come to treatment.Lesions were found in 23 cases of upper right lobe,11 cases of upper left lobe,8 cases of right lower lobe,4 cases of left lower lobe and right middle lobe.The whole group of patients with imaging findings are single isolated lesions,no distant organ metastasis.Most patients were not diagnosed before surgery,postoperative paraffin specimens confirmed pathology.50 patients underwent surgical treatment,of which 48 cases of lobectomy,2 cases of total pneumonectomy.Twenty-seven patients underwent adjuvant chemotherapy and 6 received adjuvant radiotherapy.Statistical analysis was performed using SPSS 21.0 statistical software for patient survival.P <0.05 statistically significant.K-M method for survival analysis,Cox risk model for the prognosis of single factor and multivariate analysis.Follow-up to December 2016,the median follow-up time of 38 months.Results: The overall 3-year survival rate in this study was about 38.0%and the 5-year survival rate was 14.0%.1,gender factor,5-year survival rate statistically no difference(P=0.128).There was no significant difference in the5-year survival rate between the age group and the 60-year-old group(P=0.731).3,tumor diameter less than or equal to 5.0cm and diameter greater than 5cm in patients with 5-year survival rate were statistically significant(P= 0.000)4,with or without lymph node metastasis in patients with 5-year survival rate difference is significant,statistically significant(P=0.000).5,TNM staging on the prognosis of the impact of statistical significance(P=0.002).(P=0.000),lymph node metastasis(P=0.000),TNM staging(P=0.001)was significant.While sex,age(P=0.128),(P0.731)were not statistically significant.Cox multivariate analysis,lymph node metastasis(P=0.003),TNM staging(P=0.000)were independent prognostic factors.A total of 50 patients were followed up and follow-up was performed on 50 patients with follow-up.Of these,43 had died and seven had survived,with a total 5-year survival rate of 14.0%.Conclustons: Age,sex,and postoperative 5-year survival were not associated.Patients with lymph node metastasis,TNM staging,tumor size and other factors have a more significant impact on survival. |
