ameloblastoma.Follicular pattern and plexiform pattern of ameloblastoma were collected to retrospect and performed immunohistochemistry against P63,Bcl-2,PCNA,Ki-67 and MCM2 which detected by qualitative and quantitative analysis.It was discussed that the relationship between the capsule invasion and recurrence of ameloblastoma and P63,Bcl-2,PCNA,Ki-67,MCM2 expressions.This study is to provide the experimental data and theoretical basis for clinical judgment of the prognosis of ameloblastoma.Methods:1 Clinical data: the clinical data from 60 cases of ameloblastoma were collected by paraffin embedded blocks after the surgical treatments in Hospital of Stomatology,Hebei Medical University.Patients were 28 males and 32 females,aged from 19~67 years old,with an average age of 42.5 years old.In which 12 cases were involved in maxilla and 48 cases in mandible.Ten cases of normal oral mucosa were used as the normal control.2 PV-9000 Immunohistochemical methods: instruction for the use of PV-9000 two-step immunohistochemistry kit was used in this study.3 The evaluation of positive results: P63,PCNA,Ki-67 and MCM2 were positive in the nuclei of brown granules,and Bcl-2 was positive in cytoplasm.The positive rates of P63,Bcl-2 and MCM2 were calculated,and the proliferation index of positive cells was calculated by PCNA and Ki-67.4 Statistical analysis: the experimental data were analyzed by SPSS13.0statistical software.Count data were by using X2 test,measurement data were by using T test and non parametric test were investigated.The inspection level is 0.05.Resuts:1 In accordance with the standard 2005 《 WHO Odontogenic Tumor Histological Type》,the samples were stained by HE and diagnosed as 30 cases of follicular pattern,30 cases of plexiform pattern.In which,10 cases of tumor invaded the capsules,25 cases of tumor followed recurrence after surgery.2 The expression of P63 protein in ameloblastoma and gingiva: the expression of P63 protein was positived in the nucleus.The positive cells were mainly distributed in the peripheral cells of the epithelium and the central stellate cells.There was positive expression in the spinous layer and a few basal cells.The positive expression rate of P63 in follicular ameloblastoma was lower than that of plexiform pattern(P<0.05).The positive expression of P63 in normal group was significantly different from that of in follicular ameloblastoma(P<0.05),and there was no significant difference between the plexiform pattern(P>0.05).The positive expression of P63 was significantly different between the tumor capsule invasion and postoperative recurrence(P<0.05).3 The expression of Bcl-2 protein in ameloblastoma and gingiva: the positive expression of Bcl-2 in ameloblastoma was mainly located in peripheral cube or columnar cells and a few astrocytes.Bcl-2 was positive in cytoplasm or membrane.The negative expression was found in gingiva.The expression of Bcl-2 in the follicular pattern and plexiform pattern of ameloblastoma was not statistically significant(P>0.05),and there was significant difference between the two groups(P<0.05).4 The expression of PCNA protein in ameloblastoma and gingiva: the PCNA immune markers in the nucleus and nucleolus,brownish yellow or brown.In ameloblastoma,PCNA was mainly expressed in epithelial cells and the peripheral cells and around the island and part of the stellate reticular cells.It was positive in the deep and basal layer of the gingiva.The expression of PCNA in the follicular pattern and plexiform pattern of ameloblastoma was not statistically significant(P>0.05),and there was significant differencebetween the two groups(P<0.05).5 The expression of Ki-67 protein in ameloblastoma and gingiva: the Ki-67 positive expression in the nucleus,brownish yellow or brown.The main positive region in follicular pattern was mainly located in the peripheral cells of the epithelial islands and some stellate cells.The plexiform pattern was mainly located in the peripheral cells of the terminal strip.The proliferation index of Ki-67 in follicular pattern of ameloblastoma was higher than that of plexiform pattern(P<0.05).There was significant difference between the gingiva and the ameloblastoma(P<0.05).There was no significant difference between the value added index of Ki-67 and the value added index(P>0.05)in the case of capsular invasion.In the case of recurrence,the value added index was higher than that in the cases without recurrence,and the difference was significant(P<0.05).6 The expression of MCM2 protein in ameloblastoma and gingiva: the MCM2 positive expression in the nucleus,the above expression of positive cells of the epithelium and cords peripheral cuboidal and columnar cells,stellate reticulum cells only a small amount of expression.Gingival cells in the spinous layer and basal layer of positive expression.The positive rate of MCM2 in follicular pattern of ameloblastoma was significantly higher than that of plexiform pattern(P<0.05).There was significant difference between the gingiva and the ameloblastoma(P<0.05).There was no significant difference between the value added index of MCM2 and the value added index(P>0.05)in the case of capsular invasion.In the case of recurrence,the value added index was higher than that in the cases without recurrence,and the difference was significant(P<0.05).Conclusion:1 The low expression of P63 in ameloblastoma is closely related to capsule invasion and recurrence of tumor.2 The high expression of
