The First Half And Full Use Of A Tourniquet Comparison Of Early Rehabilitation And Blood Loss After Knee Replacement

Objective Research release tourniquet after prosthesis installing and full use of a tourniquet comparison of early rehabilitation and blood loss after knee replacement.Methods This research adopts the prospectie randomized controlled study,choice on March 1,2015 to July 1,2016 admitted to the affiliated hospital of Qingdao university joint surgery diagnosis are left(or right)of knee osteoarthritis,and the first line of total knee arthroplasty(TKA).According to the different application methods of tourniquet,all patients were further categorized into experimental group(12 male cases and 38 female cases)and control group(13 male cases and 37 female cases).Respectively the 50 eligible patients were randomly divided into 2 groups:First half using a tourniquet 50 cases,full use of a tourniquet 50 cases.Record matters: time of operation,blood loss during operation,perioperative blood transfusion,the number of deep vein thrombosis of lower limbs,the number of pulmonary embolism,three days after limb week 10 cm diameter above the knee joint,VAS score 7 days after surgery,HSS score 7 days after surgery,tension blisters cases,postoperative CPM degree 3 days,on admission HCT,postoperative day volume of drainage,HCT of postoperative day,the second day after volume of drainage,the third day after HCT.And compared between groups.T-test was applied for the comparison between groups,and chi-square test was utilized for the comparison of sample rates.Statistical analysis was accomplished by SPSS 21.0 package with an acceptance criteria of P<0.05.Results Over 1 year and 4 months,all 100 subjects from joint surgery department at the affiliated hospital of Qingdao University received surgical therapy and was included in the study.Data collected via test method was calculated by professional software,the results were as following.Compared with the entire group,the first half group have longer operative time,and first and after three days of HCT value is higher,more intraoperative blood loss,less total red blood cell loss,less hidden red blood cell loss,the first and second days less volume of drainage,patients with postoperative 1 week HSS score is higher,three days after limb week 10 cm diameter above the knee joint is lower,lower postoperative VAS score 7 days,postoperative CPM degree 3 days is higher,produce fewer tension blisters,the differences were statistically significant.Two groups had no lower limb venous thrombosis and pulmonary embolism complications.Conclusion First half application tourniquet group compared to the entire application tourniquet,not only decrease postoperative recessive red blood cell loss and the total amount of red blood cell loss,but also conducive to patients with early recovery,and do not increase in the incidence of complications such as lower limb venousthrombosis and pulmonary embolism,Worth to adopt.
