Objective: to investigate the imbalance of immunoglobulin in Henoch-Sch?nlein purpura(HSP)/Henoch-Sch?nlein purpura nephritis(HSPN)and to study the level of IL-10-producing regulatory B cells in HSP/HSPN.Methods: 54 children with HSP/HSPN were enrolled into the study,and they were divided into 4 groups as follows: 16 HSP children without renal damage(Group A);13 HSPN children with only microhematuria(Group B);14 HSPN children with only proteinuria(150mg<24 hours proteinuria<50 mg/kg)(Group C).11 HSPN children with massive proteinuria(24 hours proteinuria?50mg/kg)(Group D).13 healthy children were selected as normal control group(Control).The percentages of CD19~+IgA~+B cells?CD19~+IgE~+B cells?CD19~+IgM~+B cells?CD19~+IgG~+B cells?CD19~+IgD~+B cells and CD19~+IgA~+IL-10~+B cells?CD19~+IgE~+IL-10~+B cells?CD19~+IgM~+IL-10~+B cells?CD19~+IgG~+IL-10~+B cells?CD19~+IgD~+IL-10~+B cells were detected with flow cytometry.Results: the percentages of CD19~+IgA~+B cells in Groups A?B?C and D were significantly increased comparing to those in Control group;the percentages of CD19~+IgE~+B cells and CD19~+IgA~+IL-10~+B cells in Groups B?C and D were increasingly increased compared with those in Control group;the percentages of CD19~+IgA~+B cells?CD19~+IgE~+B cells and CD19~+IgA~+IL-10~+B cells in Groups B ? C and D were increasingly increased compared with those in Group A;the percentages of CD19~+IgA~+B cells in Groups D were increasingly increased compared with those in Group B;the percentages of CD19~+IgE~+B cells and CD19~+IgA~+IL-10~+B cells in Groups C and D were increasingly increased compared with those in Group B;the percentages of CD19~+IgA~+B cells and CD19~+IgA~+IL-10~+B cells in Groups D were increasingly increased compared with those in Group C;the percentages of CD19~+IgG~+B cells?CD19~+IgM~+B cells?CD19~+IgD~+B cells and CD19~+IgE~+IL-10~+B cells?CD19~+IgM~+IL-10~+B cells?CD19~+IgG~+IL-10~+B cells?CD19~+IgD~+IL-10~+B cells showed no significant difference among each groups.Moreover,CD19~+IgA~+B cells showed positive correlated with CD19~+IgA~+IL-10~+B cells(R=0.611?P<0.001);CD19~+IgE~+B cells/CD19~+IgE~+IL-10~+B cells?CD19~+IgM~+B cells/CD19~+IgM~+IL-10~+B cells ? CD19~+IgG~+B cells/CD19~+IgG~+IL-10~+B cells and CD19~+IgD~+B cells/CD19~+IgD~+IL-10~+B cells showed no correlation.When 24 h urinary proteins reached 50mg/kg,CD19~+IgA~+B cells and CD19~+IgA~+IL-10~+B cells were increasingly increased(P<0.05,P<0.01).In HSP/HSPN with purpura,IL-10 showed positive correlated with CD19~+IgA~+B cells(R=0.268 ? P < 0.05)?CD19~+IgE~+B cells(R=0.338?P<0.05)and CD19~+IgA~+IL-10~+B cells(R=0.348? P<0.01).In HSP/HSPN with arthrosis symptom,IL-10 showed positive correlated with CD19~+IgE~+B cells(R=0.681?P<0.001).In HSP/HSPN with fecal occult blood IL-10 showed positive correlated with CD19~+IgE~+B cells(R=0.645?P<0.05).Conclusions: IgA?IgE were increased in children with HSP/HSPN.IL-10-producing regulatory B cells,especially CD19~+IgA~+IL-10~+B cells were increased in children with Henoch-Sch?nlein purpura.IL-10 was related with purpura?arthrosis symptom and fecal occult blood in HSP/HSPN.
