| Objective:To discusses the correlation between the severity of cervical pathological changes and the expression of P16 protein and L1 protein,to explore its possible in the process of cervical squamous cell cancer development state by determination of P16 and HPV L1 protein,Ki-67 expression in different cervical lesions for the clinical diagnosis of cervical squamous cell carcinoma patients and providing evidence for individualized treatment and follow-up.Methods:Collection paraffin specimens of the cervical tissue through the hysteromyoma resectionfrom or resection of the cervical lesionthe from the first hospital of shanxi medical university in pathology department.Search relevant cases at the same time to ensure that the choice of the object of study in high-risk HPV DNA testing,namely HC2 assay,and HC2(+)are confirmed in high-risk type HPV infection,pathological examination results contained the P16 protein、Ki67 and L1 protein.collect P16 protein,Ki – 67 expression and L1 shell protein expression by immunohistochemical SP method in different cervical lesions.To process the data by SPSS17.0 software for statistical analysis,categorical datas were expressed as a ratio,measurement data with((x|-)±s),The comparison of count data was performed by X2 test.The comparison between the two groups was performed by two sample t test,Multiple comparison between groups were analysised by variance analysis,test level of alph α= 0.05.Results:158 cases with cervical paraffin tissue were eligible,cervical squamous cell carcinoma of 20 cases;CINIII for 38 cases;CINII for 32 cases;CINI for 40 cases,NILM for 28 cases.1.P16 protein expression in different cervical lesions: in high-risk HPV infection in cervical tissue,positive expression rate of P16 protein in NILM CINI,CINII,CINII,cervical squamous cell carcinoma were respectively: 17.9%(5/28),20%(8/40),59.4%(19/32),100%(38/38),100%(20/20).The positive expression rate of P16 protein was on the rise with the further progress,P16 was full expressed in SCC and CINIII.2.Ki-67 expression in different cervical lesions: in high-risk HPV infection in cervical tissue,positive expression rate of Ki-67 in NILM CINI,CINII,CINII,cervical squamous cell carcinoma were respectively: 14.3%(4/28),30%(2/40),96.9%(31/32),100%(38/38),100%(20/20).The positive expression rate of Ki-67 was on the rise with the further progress,Ki-67 was highly expressed in SCC and CINIII.3.HPV-L1 protein expression in different cervical lesions: in high-risk HPV infection in cervical tissue,positive expression rate of HPV-L1 protein in NILM CINI,CINII,CINII,cervical squamous cell carcinoma were respectively: 14.3%(4/28),60%(24/40),28.1%(9/32),10.5%(4/38),0%(0/20)The positive expression rate of HPV-L1 protein was decline with the further progress,HPV-L1 protein was negative expressed in SCC.4.The temporal expression of P16 protein shell and L1 protein in cervical lesions were divided into four groups,namely the HPV L1 protein(+)/and P16 protein(-),HPV L1 protein(+)/P16 protein(+),HPV L1 protein shell(-)/and P16 protein(+),HPV L1 protein(-)/P16protein(-).The expression rate of HPV L1 protein(+)/P16 protein(-)in NILM,CINI,CINII,CINIII,SCC were: 10.7%(3/28),45%(18/40),6.2%(2/32),0%(0/38),0%(0/20);The expression rate of HPV L1 protein(+)/P16 protein(+)in NILM,CINI,CINII,CINIII,SCC were: 3.6%(1/28),15%(6/40),21.9%(7/32),10.5%(4/38),0%(0/20).The expression rate of HPV L1 protein(-)/P16 protein(-)in NILM,CINI,CINII,CINIII,SCC were: 71.4%(20/28),35%(14/40),24.4%(11/32),0%(0/38),0%(0/20).The expression rate of HPV L1 protein shell(-)/P16 protein(+)in NILM,CINI,CINII,CINIII,SCC were: 14.3%(4/28),5%(2/40),37.5 %(12/32),89.5%(34/38),100%(20/20).5.The temporal expression of Ki67 and L1 protein in cervicallesions were divided into four groups,namely the Ki67(+)/and L1 protein(-),Ki67(+)/and L1 protein(+),Ki67(-)/and L1 protein(+),Ki67(-)/and L1 protein(-).The expression rate of Ki67(+)/ L1 protein in NILM,CINI,CINII,CINIII,SCC were:10.7%(3/28),50%(20/40),3.1%(1/32),0%(0/38),0%(0/20);The expression rate of HPV L1 protein(+)/ Ki-67(+)in NILM,CINI,CINII,CINIII,SCC were:3.6%(1/28),10%(4/40),25%(8/32),10.5%(4/38),0%(0/20);The expression rate of HPV L1 protein(-)/ Ki-67(-)in NILM,CINI,CINII,CINIII,SCC were:71.4%(20/28),37.5%(15/40),0%(0/32),0%(0/38),0%(0/20);The expression rate of HPV L1 protein(-)/ Ki-67(+)in NILM,CINI,CINII,CINIII,SCC were:14.3%(4/28),2.5%(1/40),75%(24/32),89.5%(34/38),100%(20/20).Conclusion:For high-risk HPV infection in different cervical lesions,P16 protein and L1 protein expression related to the degree of cervical lesion progress,It could speculate the current state of biological process to a certain extent,in the process of development has certain significance to predict pathological changes,set up certain theoretical basis to prevent misdiagnosis and excessive medical care in the process of clinical diagnosis and treatment. |
