Comparision Of Open Versus Laparoscopic Left-sided Hepatectomy For Intrahepatic Duct Stone

Objective:Intrahepatic stone is prevalent in our own country.Although the incidence of hepatolithiasis patients declines in recent years,the population of this disease is still in a large quantity.There have three major way for the hepatectomy including pure laparoscopic,laparoscopy-assisted and the traditional open methods.The laparoscopic hepatectomy not just perform succeeded,the trend of this surgery already moved from limited resection toward major hepatectomy.But the efficiency of the laparoscopic hepatectomy still needs to be studied deeply.To discuss the efficiency of laparoscopic hepatectomy.Methods:Methods: 83 similar patients has been selected to do the research of retrospective analyze.Among them,36 cases underwent laparoscopic hepatectomy and 47 cases underwent open hepatectomies.Comparing the time of operation,bleeding volume,the time of first deflating,ambulation,the period of hospital staying,the clearance and recurrence of stone.Results:The laparoscopic hepatectomy operation time(232.0±45.6min)is longer than the open hepatectomy group(180.0±21.4min)and the P﹤0.05.But the shorter time of deflation and ambulation time,which is [(2.0±0.5d)vs.(3.5±0.7),P﹤0.05](1.6±0.4d)vs.[(4.0±0.7d)P﹤0.05].the bleeding volume of the laparoscopic hepatectomy(320.0±187.5mL)also smaller than the open hepatectomy(396±223.7mL).The clearance rate is(88.9% vs.89.4%)and relapse rate is(97.2% vs.100%).There is no difference about the clearance and relapse rate between two operation method,P>0.05.Conclusion:According all the selected patients’ data,the efficiency of laparoscopic hepatectomy has no different with the open hepatectomy.This is a quiet safe operation like traditional one and it has many other advantages such as little gastrointestinal disturbance,small incision and quick recovery.
