Objective To detect the resting state functional MRI(rf MRI)changes and its relationship with the clinical curative effect of anti-epileptic drugs of the complex partial seizures(CPS)epilepsy patients before and after treatment by employing the fractional amplitude of low frequency fluctuation(f ALFF).Methods 14 patients with CPS were collected from the Department of Neurology of the Affiliated Hospital of Inner Mongolia Medical University from June 2015 to June 2016.These patients were investigated by comparing with 14 gender,age,handedness and normal education matched healthy subjects.All subjects were tested with GE Discovery 750 3.0T superconducting magnetic resonance scanner.The conventional sequences include the T2 WI,T1WI,in order to exclude intracranial organic lesions,the function sequences include T1-3D BRAVO,BOLD.The patients were all ate the first-line antiepileptic drugs,tested f MRI again after 12 weeks.The data of the cases before and after treatment were compared with those of healthy controls.Results superimposed on the standard avg152T1 image display,take the threshold P=0.001(uncorrected),after Alphasim correction,removal of less than 25 of the area of the individual(corrected P < 0.001),get the difference between the two groups of subjectsResults Compared with the normal control group,the Before treatment brain area of fALFF value was significantly higher in the bilateral thalamus,posterior cingulate gyrus and the left lateral horn.Compared with the CPS patients before treatment,the patients after treatment showed abnormal brain activation areas disappeared in the bilateral thalamus or decreased in the bilateral anterior cingulate gyrus and the left angular gyrus.The left inferior parietal cortex activation scope larger than before.The bilateral frontal lobe,precuneus and the left cuneus was been found new brain activation areas(p<0.001,cluster size≄25).Conclusion 1.fALFF can observe the brain functional changes in primary patients with epilepsy before and after treatment,provide new insights for the clinical to evaluation the treatment and the effecting of anti-epileptic drugs,it can also provide new ideas for the pathophysiological mechanism.2.There is a wide range of abnormal activation and inhibition of brain regions in patients with primary epilepsy,which is consistent with the previous literature,and which is considered the neural basis of epilepsy.3.After 12 weeks of antiepileptic treatment,some abnormal brain regions of CPS patients returned to normal,suggesting that the abnormal changes of brain function in patients with primary epilepsy can be reversed by anti epileptic treatment.4.After the treatment,we found a small part of the non reversible area and slightly enlarged activation area in the left inferior parietal lobule,the pathophysiological mechanism needs us further study.
