| Objective: To study the Levonorgestrel Intrauterine System non contraceptive benefits in the treatment of gynecological diseases(menorrhagia,dysmenorrhea and uterine adenomyosis).Method:According to the inclusion and exclusion criteria,patients who were inserted the LNG-IUS in Family planning clinic of Tianjin Medical University General Hospita were recruited.According to the data of medical history and the results of gynecological ultrasound,the patients were divided into the group with increased menstruation,dysmenorrhea,adenomyosis.The patients were followed up for 12 months.SPSS19.0 was used to record and analyze the data of the groups,and the descriptive analysis was used to calculate the mean value,standard deviation,composition ratio and rate.The changes of each group before and after placement of LNG-IUS in each group were compared by paired data t test.Test levelα=0.05(bilateral).Results: 1.A total number of 145 patients were recruied.All patients were followed up for 12 months,with a total of 140 cases,including 44 cases of menorrhagia,48 cases of dysmenorrhea,48 cases of adenomyosis.2.Study on sub groups of subjects increased menstrual mean age was 40 + 7.17 years of age,gravidity and parity mean were 2.8 + 1.29 and 1.3 + 0.70 times,placed in the pre LNG-IUS days of menstrual period,the mean of 44 subjects was 6.67 + 1.88 days of menstruation,mean 147.27 + 50.92 ml,blood red protein the mean concentration was 112.11 + 18.97g/L.Comparison of the number of days of menstruation,menstrual volume and hemoglobin levels of the patients with idiopathic menorrhagia at months 6,12 after insertion of LNG-IUS,the number of menstrual period and menstrual volume were reduced,and hemoglobin level was increased,the difference was statistically significant(p<0.05).3.Study on sub groups of subjects increased menstrual mean age was 40 + 7.17 years of age,gravidity and parity mean were 2.8 + 1.29 and 1.3 + 0.70 times,placed in the pre LNG-IUS days of menstrual period,the mean of 44 subjects was 6.67 + 1.88 days of menstruation,mean 147.27 + 50.92 ml,blood red protein the mean concentration was 112.11 + 18.97g/L.Before and after the placement of LNG-IUS for 3,6,12 months,the VMS scores of dysmenorrhea were decreased,and the difference was statistically significant(p<0.05),the number of days of menstrual cycle was decreased,the difference was statistically significant(P < 0.05)4.Adenomyosis of sub group included 50 subjects,including 21 subjects also underwent MRI examination.In the follow-up to 6 months and 12 months,respectively 1 subjects lost in 12 months,there were 12 subjects underwent MRI scan.The mean uterine adenomyosis study subjects age is 41.38 + 5.04,3.19 + 1.36 times the mean gravidity,parity mean was 1.29 + 0.71 times,the menstrual cycle mean was 29.46 + 4.46 days,mean 6.44 days of menstrual period 2.28 days.For adenomyosis patients,before and after the placement of LNG-IUS for 3,6,12 months,days of period,the VMS score and menstrual volume were reduced,and the level of hemoglobin was increased,the level of serum CA125 decreased,the difference was statistically significant(p<0.05).Although the mean value of uterine volume after LNG-IUS implantation 3months,6months and 12 months was less than that before placement,the difference was not statistically significant(p=0.501,0.835,0.097,respectively).The thickness of junctional zone and endometrium before and after the placement of LNG-IUS 12 months had a significant difference(p < 0.05).Conclusion:1、LNG-IUS can effectively reduce the amount of menstrual blood loss and improve the level of hemoglobin.2、LNG-IUS can reduce the dysmenorrhea score,effectively alleviate the symptoms of dysmenorrhea.3、LNG-IUS can significantly improve the menstrual cycle,dysmenorrhea and anemia in patients with adenomyosis. |
