Objective:In order to explore the basic situations of hospitalized leukemia patients with initial treatments,remissions after initial treatment of acute leukemia patients and the prognosis factors,we collected hospitalized records of leukemia patients in hematology department of a 3A degree hospital in Qiqihar City.The outcome of this study can provide a scientific basis for controlling the occurrence and development of leukemia,guiding the tertiary prevention and disease surveillance of local leukemia patients,so as to assist health departments to make decisions.Methods:Hospitalized leukemia patients were retrospectively analyzed from April 2014 to April 2017 in hematology department of a 3A degree hospital in Qiqihar City.Epi Data3.1 software was used to establish databases and SPSS21.0 software was used to analyze data.The basic situations of leukemia patients,including basic demographic characteristics,past history and living habits,were described by constituent ratio.The quantitative variables were described by if X±S they obeyed the normal distribution,otherwise they were described by M(P25,P75).The difference of quantitative variables between groups was measured by Mann-Whitney U test.The ratio and constituent ratio were used to describe qualitative variables,and chi square test or Fisher’s exact test was used to measure the difference of qualitative variables between groups.Logistic regression was used for multivariate analysis.P < 0.05 was considered of statistical significance.Result:1.A total of 233 hospitalized leukemia patients were included in our study,including 151 AL ptients(64.8%),56 CL patients(24.0%)and other 26 cases(11.2%);The died cases were 71 while survival cases were 159,3 cases were deletion.2.Of all the included patients,130(55.8%)cases were male and 103(44.2%)cases were female,the sex ratio was 1.26:1;The range of age was from 9 to 89,the average age was 54.3±17.2 years;the Han nationality was the most common nationality,accounting for 97.4%;The married patients were 193(82.8%)cases,accounting for the vast majority;134 patients(57.5%)were outside the county;Among all occupation,87(37.3%)patients without any occupation were the most common,followed by 66(28.3%)farmers;The past history of patients included hypertension,heart disease and blood clots,accounted for 17.2%,12.0% and 11.6%,respectively;100 patients had bad living habits,the proportion was 42.9%,of which 46(46.0%)cases had smoking history and 30(30.0%)cases had drinking history.232(99.6%)cases had initial symptoms,including 180(77.3%)cases with anemia,100(50.5%)cases with consumption symptoms and 120(42.7%)cases with infiltration symptoms(42.9%).The ECOG scores before treatment of 179(76.8%)cases were less than 2 scores.3.Among the 233 included patients,151(64.8%)cases were acute leukemia(AL),including 27(11.6%)acute lymphoblastic leukemia(ALL)and 124(53.2%)acute myeloid leukemia(AML);56(24.0%)cases were chronic leukemia(CL),including 39(16.7%)chronic myeloid leukemia(CML)and 17(7.3%)chronic lymphocytic leukemia(CLL);26(11.2%)patients had other types of leukemia.The subtypes of AL mainly were M2,M3 and M4,accounting for 33.1%,19.4% and 20.2%,respectively.4.The age of AL patients ranged from 9 to 89 and the average age was 51.9±18.3 years,there were significant difference in the remission after initial treatment of different age groups and marital status(P<0.05).There were significant difference in the remission after initial treatment of patients with hypertension history and the history of blood clots(P<0.05),there were significant difference in the remission after initial treatment of patients with different ECOG scores(P<0.05),and the remission of patients with low scores were better than patients with high scores.The white blood cell(WBC),chromosome karyotype,gene mutation had statistically significant effort on the remission after initial treatment(P<0.05).5.The initial symptoms was anemia in 118(78.1%)AL patients while secondary infection in 68(45.0%)cases,which had statistically significant effects on the prognosis of AL patients(P<0.05);Meanwhile,the age and ECOG scores had statistically significant effects on the prognosis of AL patients(P<0.05).6.Organ failure was the main cause of death in hospitalized leukemia patients,accounting for 38.0%,followed by bleeding,accounting for 29.6%,the primary recurrence and infection accounted for 9.9% and 11.3%.Conclusion:1.Among the hospitalized leukemia patients in our study,the incidence of male is slightly higher than that of female;the majority of subtype is adult AML;the majority of patient are outside the county in the spatial distribution,and the occupations are mainly unemployed and farmers.2.Age,ECOG score,hypertension history,thrombosis history,initial symptom hemorrhage,white blood cell(WBC),chromosome karyotype,gene mutation,are the influence factors of remission after treatment in AL patients.3.Age,anemia,secondary infection and ECOG score may be the prognostic factors of AL patients.4.Organ failure,bleeding and infection are the main causes of death of leukemia patients.
