[Objectives] This study compared the effects of delayed cord clamping and immediate cord clamping on preterm infants with vaginal delivery.With comparison of immediate cord clamping,it explored the benefits and risks of delayed cord campling for preterm infants with vaginal delivery.The writer hopes that this study can discover the relation between methods of cord clamping and prognosis of premature infants,in order to provide a basis for the prognosis of vaginal delivery premature infants and improve the quality of life of premature delivery.[Methods] Expected vaginal delivery of 27-35 weeks premature infants and their mothers were selected as research crowd,during July 2015 to December 2016 in Wuxi City Maternity and Child Hospital.(After getting agreed by the pregnant women and their families,)according to the randomized control principle,200 cases,according to the condition of inclusion,were selected and divided into delayed cord clamping group(DCC Group)and immediate cord clamping group(ICC group).The indexes were respectively observed in these two groups of premature infants,including hemoglobin(Hb),haematocrit(Hct),blood gas analysis,coagulation function,transcutaneous bilirubin(TCB),periventricular-intraventricular hemorrhage(PIVH),clinical complication and etc.Data were collected and analyzed with SPSS17.0 statistical software.[Result] The values of HB and HCT in preterm infants with delayed cord clamping group(DCC Group)were significantly higher than that of the immediate cord clamping group(ICC Group)three days after delivery(p<0.05);The partial pressure of Oxyge(PO2),the standard alkali residue(sBE)and the average arterial pressure of DCC group were higher than the ICC group(P<0.05)in the blood Gas analysis;the PIVH incidence and apnea rate of premature infants(p<0.05)could be significantly reduced in DCC group,same situation for the end-stage blood flow loss,cerebral arterial pulsation and resistance index in premature infants and the occurrence of liquid dark region in the brain(P<0.05).However,no significant differences happened in the incidence of coagulation,TCB Peak,phototherapy and Hyperbilirubinemia(P>0.05)between the two groups.[Conclusion] Delayed cord clamping can increase the level of hemoglobin and hematocrit for preterm infants,increase the blood volume of preterm infants,and reduce the rate of anemia and blood transfusion during hospitalization.It can also improve the oxygen partial pressure and the residual value of the standard alkali in preterm infants and reduce the incidence of apnea.Delayed cord clamping can effectively reduce the incidence of intraventricular hemorrhage,reduce the loss of blood flow in the middle cerebral arteries,the pulse of cerebral arteries and the high resistance index of premature infant,and prevent the occurrence of intracranial hemorrhage in the brain.Moreover,it does not increase the risk of jaundice and phototherapy.Therefore,it is an economical,safe and simple clinical treatment method,which is important for improving the quality of life of premature infants...
