| Object: By studying the effects of Xiaorong Wenban Fang on Qi deficiency phlegm and blood stasis in patients with unstable angina pectoris serum IL-18 、 TGF-β1 inflammatory factors,to explore the anti-inflammatory effect of Xiaorong Wenban Fang on coronary unstable plaque.Methods: Taking clinical observation method,the clinical case report form was formulated and analyzed,and the outpatient and hospitalization of Geriatrics and department of cardiology in the first affiliated hospital of Henan university of traditional Chinese medicine in 2016 were selected for diagnosis: chest obstruction and blood stasis;Western medicine diagnosis: patients with unstable angina pectoris of coronary heart disease,a total of 60 cases.The patients were randomly divided into two groups according to random number table,The control group was given western medicine standardized anticoagulation,anti platelet aggregation,anti ischemic,lipid-lowering oral [Atorvastatin Calcium Tablets(Li Putuo),20 mg,before sleeping once] treatment,the observation group based on Western medicine standardized treatment combined with Chinese medicine granule Xiaorong Wenban Fang.Observation of two groups of patients with Chinese and Western medicine,blood lipids and serum inflammatory factors IL-18,TGF-β1 levels,and the characteristics and volume changes of plaque were measured by 64-slice spiral CT.Through the statistical software package spss17.0 to establish the database,data management and analysis,and finally get the results.Results: 1.before treatment,there was no difference between the two groups(p > 0.05);2.after treatment,the efficacy of Chinese and western medicine,blood lipid,TGF-β 1,IL-18,hs-CRP,the properties and volume of coronary plaque compared with before treatment,there was significant difference(p < 0.05);3.after treatment,the observation group compared with the control group:(1)the symptoms of angina pectoris in the observation group improved 15 cases,effective 13 cases,ineffective 2 cases,total effective rate 93.33%,control group 9 cases,effective 11 cases,10 cases,the total effective rate was66.67%;In the improvement of TCM syndrome integral,the syndrome score of observation group and control group decreased,and had significant difference(p < 0.05),the improvement of observation group was better than the control group.There was no significant difference between the two groups(p > 0.05).Patients in both groups did not show the exacerbation of angina symptoms.(2)blood lipid: the TC,TG,LDL-C of the observation group were lower than that of the control group,and HDL-C was significantly higher than the control group(p < 0.05);(3)xieqingyanzheng factor: the level of IL-18 in observation group was lower than that in control group,and the level of TGF-β 1 was significantly higher than control group(p < 0.05);(4)dual source spiral CT: Nature of plaque: the number of Soft plaque in the observation group decreased significantly(p <0.05),the number of fibrous plaque and hard spot increased,but there was no significant difference(p > 0.05);In the control group,the number of Soft plaque decreased,but there was no significant difference(p > 0.05),the number of fibrous plaque and hard spot increased,but there was no significant difference(p > 0.05);Intergroup comparison: the number of Soft plaque in the two groups decreased,the number of fibrous plaque and hard spot increased,the observation group decreased significantly than the control group(p <0.05).Plaque volume: two groups before and after treatment decreased significantly,with significant difference(p < 0.05).Conclusion: Xiaorong Wenban Fang combined with atorvastatin can increase the level of TGF-β 1 in patients with unstable angina pectoris,reduce the level of inflammatory cytokines such as IL-18,and help to stabilize coronary plaque,give full play to anti-inflammatory and anti-inflammatory effects;To alleviate the symptoms of myocardial in patients with unstable angina pectoris and improve the clinical prognosis of patients with unstable angina pectoris;Both have the synergistic effect of treatment,for the clinical treatment of unstable angina,stable coronary unstable plaque,effectively reduce the incidence of unstable angina pectoris of cardiovascular events provide a more reliable theoretical basis. |
