Obejective To study preoperative and postoperative auditory and verbal ability of 467 Cochlear implanted children subsidized by Chinese and Hebei deaf people rehabilitation center in Hebei province,and analyze which factors can affect the improvement of auditory and verbal ability after Cochlear implantation.Thus we can provide better services and policies to help those hearing impaired poor children.Methods We analyzed the preoperative and postoperative one year audibility and speech ability of 467 cochlear implanted deaf children(younger than 7 years old)from 2013-2015.Further we studied which factors can affect the recovery of hearing and speak ability.All enumeration data were analyzed through rate and constituent ratio,and the postoperative affected factors were analyzed by rank sum test and Logistic regression analysis.All data were input in Epi Data3.1 database and analyzed by SPSS20.0.Results 1.There are 467 cochlear implanted deaf children from 2013-2015,including 258 boys(55%)and 209 girls(45%);146 children are younger than 3 years old(31%),288 at 4 to 7 years old(69%);139 children used hearing aid less than 6 months(30%)and 328 longer than 6 months(70%).162 children were implanted Austrian cochlear(35%),103 children were implanted American cochlear(22%),and 228 children were implanted Australian cochlear(43%).After surgery,318 children did auditory and verbal rehabilitation training in Special-education schools(68%),14 inhome(32%).2.Preoperatively,no child have the first level of hearing recovery auditory ability(Average speech recognition rate)(0%),2 children have the second level of hearing recovery auditory ability(0.5%),6 children have the third level of hearing recovery auditory ability(1.3%),74 children have the fourth level of hearing recovery auditory ability(15.8%),and 385 children did not reach up to recovery standard(82.4%).Postoperatively one year,155 children obtained the first hearing recovery auditory ability(33.2 %),97 children got the second hearing recovery auditory ability(20.8%),60 children reached the third hearing recovery auditory ability(12.8%),91 children had the fourth hearing recovery auditory ability(19.5%),and 4 children were not up to recovery standard(13.7%).Preoperative average speech recognition rate was 24.3%,postoperative was 74.5%,and postoperative average speech recognition rate was better than Preoperative.3.Preoperatively,100 children were one year old verbal age(21.4%),66 children were two year old verbal age(14.1%),66 were three year old verbal age(14.1%),and 11 were four year old verbal age(2.4%),and 224 were not up to verbal age standard(48%).Postoperatively one year,6 were one year old verbal age(16.3%),148 children were two year old verbal age(31.7 %),157 were three year old verbal age(33.6%),32 were four year old verbal age(6.9%),and 54 were not up to verbal age standard(11.5%).Preoperative average verbal age was1.2,postoperative was 2.5,and postoperative average verbal age was better than Preoperative.4.According to one-way anova and logistic regression analysis,implanted age,time of hearing aid used,and rehabilitation mode can affect postoperative auditory and verbal ability improvement.Conclusions 1.After cochlear implants were implanted one year,auditory and verbal ability of the poor profound sensorineural deafness children in He Bei province were improved effectively.2.Implanted age,time of hearing aid used,and rehabilitation mode can affect postoperative auditory and verbal ability improvement.3.Present helping policies for poor profound deaf children to implant cochlear in Hebei province are right and practical.According to our data,we suggested profound sensorineural deafness children should use hearing aids and cochlear implants early.After cochlear implants were implanted,every children should receive different hearing and speak rehabilitation according their own situation...
