| Objective : To evaluate the curative effect of catheterization sclerotherapy in combination with direct puncture cirsenchysis for lower extremity varicose veins.Methods : The clinical data of 60 patients(93 legs)with primary lower extremity varicose veins were retrospectively analyzed in our hospital from January 2012 to January 2014.For the reasons of disease stage,indifferent health and poor economic conditions,some of these patients were not suitable for surgical operation and tiny traumatic interventional treatment,and others reject these therapies.All patients with preoperative lower extremity deep vein angiography examination,were eliminated disorder of lower extremity deep venous reflux,and were diagnosed with primary lower extremity varicose vein.According to different way of foam sclerotherapy,30 patients(53 legs)underwent simple direct puncture cirsenchysis(control group),while the other30 cases(40 legs)accepted catheterization sclerotherapy in combination with direct puncture cirsenchysis(joint group).The two groups of patients had no significant statistical difference in their preoperative gender,age,course and the preoperative clinical condition,so the comparison was viable.The day after catheter-injected treatment,the subjects who had residual varicose veins had been treated by anterograde inserting to the varicose veins for injection treatment.The performance comparison would be conducted in these aspects of sclerotherapy dosage,medical expense,treatment time,clinical disease,curative effect,complications and recurrence rate.Result:The sclerotherapy was successfully accomplished in all affected patients.The average dose of lauromacrogol for therapy group were(6±4.6)ml and the control group were(12±6.2)ml.Meanwhile,the surgery time of therapy group were 52.5±13.6mins.And the time of control group were(35.0±12.5)mins.The medical expense of therapy group were(6895±364.1)yuan.And the medical expense of control group were3573±236.7 yuan.These results were comparable for two groups for statistical significance(p<0.05).And the clinical performance of therapy group were better than that of control group after 3 and 6 months by the Z-test conclusions(p<0.05).It was verified that the treatment effect was become better over time.And the recurrence rate of therapy group was less than that of control group(p<0.05).There were no complications in both groups.Conclusions:The simple direct puncture cirsenchysis had the advantages of minimally invasive,safe,shorter operation time,lower operation fees,less complications,lower recurrence rate,and effective treatment.Compared with the simple method,the method of catheterization sclerotherapy in combination with direct puncture cirsenchysis can improve the curative effect,reduce the dosage of curing agent,reduce the incidence of complications,but the operation time were prolonged,and the fees were higher.The two methods have advantages and disadvantages in the treatment for lower extremity varicose veins,the therapeutic scheme should be individualized for each patien. |
