Study Of Relationship Between The Changes Of Interleukin-6、Tumor Necrosis Factor-Alpha Levels And Enthyroid Sick Syndrome In Maintenance Hemodialysis Patients

Background and objective:The clinical incidence of euthyroid sick syndrome is high,especially in the critical patients,low T₃ syndrome is the most common.One of the mechanisms is the inflammatory factors inhibitory the 5’-monodeiodinase activity,causes the T₄ to T₃ conversion barrier.The microinflammatory state exists in the patients with end-stage renal disease,causes thyroid dysfunction in non-thyroid diseases.Effects of maintenance hemodialysis on microinflammation has different argument,some scholars think the maintenance hemodialysis increase the development of inflammatory responses,because the dialysis membrane affects the body’s inflammatory responses.Other scholars think the maintenance hemodialysis decrease the cytokines levels of the peripheral blood,by adsorbing and removing cytokines,improve the microinflammatory state.This study was conducted by observation the thyroid function and interleukin-6、tumor necrosis factor-αlevels before hemodialysis and after maintenance hemodialysis in end-stage renal disease complicated with ESS patients,To investigate the relationship between IL-6、TNF-αlevels and enthyroid sick syndrome in hemodialysis patients.Methods:1、The patients with end-stage renal disease admitted to the Department of Nephrology,94 Hospital of Chinese People’s Liberation Army from July 2015 to August 2017,complicated with ESS were divided into experiment group,and uncomplicated with ESS were divided into control group.According to the inclusion and exclusion criteria,67 subjects were included.2、Detected the thyroid function and IL-6、TNF-αlevels in the experiment group and the control group.3、Further detected the thyroid function and IL-6、TNF-αlevels before hemodialysis and after maintenance hemodialysis（at least 6 months）in the patients of the experiment group.Results:Patients with end-stage renal disease complicated with ESS have high incidence,the level of IL-6、TNF-αin end-stage renal disease complicated with ESS patients significantly increased,the level of IL-6、TNF-αafter hemodialysis is lower than before hemodialysis,the state of ESS is also improved.Conclusion:The occurrence of ESS is related to the increase of inflammatory factors（IL-6、TNF-α）,no drug intervention is recommended,with the stability of the disease,ESS can be alleviated.
