Effect Of Clinical Hunt-hess Ⅳ-Ⅴ Grade Aneurysmal Subarachnoid Hemorrhage Patients Underwent Endovascular Surgery

Objective: To investigate endovascular surgery with continuous lumbar drainage clinical efficacy Hunt-Hess Ⅳ-Ⅴ grade aSAH therapy in patients with subarachnoid hemorrhage and observe its effect on the prognosis.Methods: According to analyze the clinical diagnosis and treatment information of the 84 cases of Hunt-Hess Ⅳ-Ⅴ grade aneurysmal subarachnoid hemorrhage in patients in our hospital,two groups of patients do the endovascular embolization surgery and conventional treatment within 3 days.Among them,42 cases underwent routine lumbar puncture,which was the control group.The other 42 cases were treated by continuous lumbar cistern drainage,which was the treatment group.In order to evaluate the degree of pain before and after treatment by Visual simulation(VAS);to evaluate the prognosis of the patients by Glasgow Coma Scale(GCS),to detect the The velocity of the middle cerebral artery(Vm)of two groups of patients before treatment and after treatment,the methods are followed,to add up the statistic for the clinical efficacy and the time for intracranial pressure to restore the normal time and the time for headache relie;to add up the Statistics of two groups of patients during the treatment of complications occurred.Results:The treatment group clinical total efficiency of 76.19% was significantly higher than 40.48% of control group,P<0.05;treated intracranial pressure returned to normal time and headache remission time was(13.1±2.1)d、(9.2±1.0)d,They were significantly shorter than the control group(15.1±2.8)d 、(14.4±1.8)d,P<0.05.Treated hydrocephalus and cerebral vasospasm complications were significantly lower than the control group,P<0.05;after treatment,both groups VAS score,and Vm significantly decreased compared with prior treatment,GCS score rised compared with prior treatment,then the treatment group indicators the degree of change more significant than the control group,P<0.05.Conclusion: Embolization within continuous lumbar drainage joint blood vessels can effectively reduce the incidence of cerebral vasospasm,communicating hydrocephalus of Hunt-Hess Ⅳ-Ⅴ grade aSAH patients and other complications can be improved Hunt-Hess Ⅳ-Ⅴ grade aSAH clinical prognosis.
