OBJECTIVE: To study the possible mechanism and efficacy of 308 excimer light,halometasone and combination of the two treatments in the treatment of vitiligo.METHODS: 34 normal brown guinea pigs in the back of 4x4 cm size skin area,daily smear 5% hydrogen peroxide twice,for 60 days,to the hair removal area of skin and hair white,randomly take a guinea pig which back white.And then the remaining 32 guinea pigs of vitiligo were randomly divided into four groups: model group,308 nm excimer light group,Halometasone group,and 308 excimer light combined with halometasone group(here in after referred to as the combined group).The model group was not treated;the 308 excimer light group was treated with light;the 0.05% halometasone ointment was applied to the halometasone group;the combined group was irradiated with 308 excimer light,and 0.05% halometasone ointment was applied.All the experimental groups were treated for 5 weeks.At the end of the experiment,the levels of tyrosinase were measured by ELISA in peripheral blood of guinea pigs.The skin of the back experimental area was taken for pathologic section.The pigmentation was evaluated by HE staining and the melanin formation was observed by Masson-Fontana method.The proliferation of melanocytes was observed by the method of Imokawa staining.Result:1.The number of melanocytes of the 308 excimer light group,the Halometasone group,the combined group were compared with the model group of one,the difference was statistically significant(p<0.05).The number of melanocytes of the 308 excimer light group and the Halometasone group were respectively compared with the combined group of one,the difference was statistically significant(p<0.05).The number of melanocytes of the 308 excimer light group was compared with the Halometasone group,the difference was not statistically significant(p>0.05).2.The number of melanin granules of the 308 excimer light group,the Halometasone group,the combined group were compared with the model group of one,the difference was statistically significant(p<0.05).The number of melanin granules of the 308 excimer light group and the Halometasone group were respectively compared with the combined group of one,the difference was statistically significant(p<0.05).The number of melanin granules of the 308 excimer light group was compared with the Halometasone group,the difference was not statistically significant(p>0.05).3.The tyrosinase content of the 308 excimer light group,the Halometasone group,the combined group were compared with the model group of one,the difference was statistically significant(p<0.05).The tyrosinase content of the 308 excimer light group and the Halometasone group were respectively compared with the combined group of one,the difference was statistically significant(p<0.05).The tyrosinase content of the 308 excimer light group was compared with the Halometasone group,the difference was not statistically significant(p>0.05).Conclusion:308 excimer light,Halometasone,and 308 excimer light combined with Halometasone can effectively promote melanin synthesis in the guinea pig model of vitiligo,in which the Combined use is more significant than the single use.
