Analysis On The Effect Of Comprehensive Reform Of County-level Public Hospitals In Jiangsu Province

Objectives:In order to understand the effect of comprehensive reform of the first batch of county-level public hospitals in Jiangsu Province,this study investigates 14 counties of Jiangsu Province in the manpower situation,health facilities,medical services,expenses,assets and liabilities and the situation of income and expenditure of 26 county-level public hospitals in 2012 and 2016.It also evaluates the existing problems of comprehensive reform of county-level public hospitals in Jiangsu Province.At the same time,the study provides a more favorable scientific and useful reference for the comprehensive reform of public hospitals at county level in Jiangsu Province.Methods①Literature analysis.Through consulting relevant data,we collate and summarize the current research status of public hospitals at home and abroad,especially the implementation of the reform policy of county-level public hospitals.Then we would lay a theoretical foundation for the design of the project,the determination of the research focus and the writing of the review.②Groups.We combined with the data and organize members to further study the scheme,key indicators and analysis methods,then modify and determine the final thesis framework and guideline opinions for this paper.③Expert consulting method.In order to evaluate effect of the reform,We consulted 20 experts repeatedly and established evaluation index system of public hospitals at the county level in Jiangsu province finally.④Statistical analysis.We adopted the proportion and growth speed to analysis the gap of jiangsu 26 county-level hospitals in the southern of jiangsu,the middle of Jiangsu and the northern of Jiangsu in 2012 and 2016.⑤Data envelopment analysis(DEA).We Use DEAP2.1 to evaluate hospital efficiency of pilot counties of jiangsu province.⑥Comparative research.This study compared 26 pilot reform of public hospitals to analysis the healthy effects of human resources,health facilities,medical services,medical expenses,assets and liabilities,income and expenditure.Results:After the reform,the overall operation of first group of 26 county-level public hospitals in Jiangsu Province was sound:the volume of service increased year on year,the service capacity,medical resources and the ability to repay debt were all effectively enhanced.However,there were still a lot of problems to be solved:the production efficiency of county-level public hospitals was declining,the operating burden was heavy,the financial subsidy was insufficient and the regional gap was still large.The service efficiency and hospital income and expenditure structure need to be improved,and the effect of difference in medicine also need to be optimized.Suggestions:Firstly,we should optimize the allocation of medical resources and improve the utilization efficiency of county-level public hospitals.Secondly,we should attach the importance to the construction of talent teams and improve the quality of health technicians.We will continue to actively adjust the price of medical services and promote the transformation of medical insurance payment.Then we should control the debt scale of county-level public hospitals reasonably and strengthen the management of financial risk control.We will further improve the financial compensation mechanism and strengthen comprehensive supervision.Finally we need to strengthen regional cooperation and improve the hierarchical medical system.
