| Objective: To observe the clinical effect of lumbar sympathetic nerve block combined with traditional Chinese medicine in the treatment of osteonecrosis of the femoral head(ONFH).We observed the changes of the patients’ condition before and after treatment so as to explore the control and curative effect of this treatment for osteonecrosis of the femoral head.Methods: To collect bone Department II TCM Hospital of Wuhan city from March 2016 to December 2017 32 cases(male 14 cases,female 18 cases(54 hips)bilateral disease in 22 cases,6 cases,the incidence of right 4 cases)patients with femoral head necrosis were clear,all the patients were randomly divided into treatment group and the control group,the treatment group of 16 cases(male 6 cases,female 10 cases),the control group of 16 cases(male 8 cases,female 8 cases).Two groups of patients were required to strictly rely on crutches to reduce hip bearing,and during the whole treatment,patients were asked to discontinue other drugs or treatments directly related to osteonecrosis of the femoral head.The treatment group was given lumbar sympathetic nerve block for treatment with traditional Chinese medicine treatment,control group oral Chinese herbal decoction.Continuous observation after 3 months of follow-up,record the basic conditions of the patients,before treatment,3 months after the treatment of patients with radiographic examination,analysis of records of the necrotic area and score before and after treatment were 3 months,1 weeks and 1 months after treatment,after treatment and record the two groups of patients with VAS the pain score,Harris score,by statistical analysis of all data obtained.Results: All two groups of patients completed this clinical study and provided a complete data record.After treatment,the treatment group in the cases group,VAS score before treatment and 1 weeks after treatment of P<0.05,have obvious difference,and 1 weeks after treatment after treatment during 3 months VAS score compared with P>0.05,no significant difference,indicating that the treatment group scheme can relieve the pain in a short period of time,and maintain the pain level in a longer period of time.Compared with the control group the case group in the VAS score before treatment and after treatment of 1 weeks than P>0.05,no significant difference,compared with P<0.05 for 1 months to 3 months after treatment during the follow-up data after treatment were significantly different,indicating the control group scheme in the short term effect is not obvious,persisted after 1 months of pain decreased.The VAS scores of the two groups were compared with P<0.05,which showed significant difference,indicating that the treatment group was better than the control group in alleviating pain.In terms of Harris score,there was a significant difference between the two groups in each time group,showing that the treatment group had better effect on improving the function of the hip than the control group in the P<0.05 group.In the area of necrotic area,there were significant differences in P<0.05 between the two groups before and after treatment,indicating that the treatment group was better than the control group in the control of necrosis.In the curative effect,the total effective rate of the treatment group was 93.75%,which was significantly better than that of the control group(56.25%),and the difference was statistically significant(P<0.05).Conclusion: Lumbar sympathetic nerve block combined with traditional Chinese medicine in the treatment of femoral head necrosis effect is obvious,can quickly relieve pain in the short term,and can maintain a low level of pain for a long time,can significantly improve the function of hip joint activity,also has a certain effect to control for the development of necrosis,is a worthy hip preserving treatment clinical application. |
