BackgroundPrimary hepatic carcinoma is one of the most common malignant tumors in China. The fatality rate of the primary hepatic carcinoma resides in second of the malignant tumors.Primary liver cancer is a malignant tumor with high malignancy, rapid progression, poor prognosis, and aggressive disease. The recurrence rate of advanced liver cancer is high and the metastasis is easy.Early diagnosis is very important. The serum markers of primary hepatic carcinoma are numerous. There is no single marker that is sensitive and specific for the diagnosis of all primary hepatic carcinoma.AFP is a kind of embryo specific alpha globulin found in fetal serum in 1956.In normal conditions, the alpha-fetoprotein,which is present in early fetal serum, disappears rapidly after birth. If it is reproduced in adult serum, the possibility of liver cancer is suggested. Therefore, the serum alpha fetoprotein is the most commonly used tumor marker in the diagnosis of liver cancer and is widely used in the early diagnosis of liver cancer. It is certain that AFP not only can be used to diagnose liver cancer, but also can monitor the recurrence, and it can be used as a reference standard for the efficacy of primary liver cancer. But it is not accurate to determine whether the prognosis and the condition of the disease can be judged.PurposeTo discuss the relationships of alpha fetoprotein(AFP)with tumor pathology grade, clinical stages and prognosis of the hepatocellular carcinoma.MethodUsing the immumofluorescence method to examine the concentration of AFP in 112 hepatocellular carcinoma patients’ serum before the operation,to analysis and comparison with tumor pathology grade, clinical stages and prognosis.Results1.The smaller the diameter of the tumor, the lower the preoperative serum AFP level(P<0.05).2.There was no correlation between the clinical stage of liver cancer and the level of serum AFP expression before operation(P>0.05).3.The higher the tumor pathology grade, the lower the preoperative serum AFP level(P<0.05),and the lower the pathology grade, the higher the preoperative serum AFP level(P<0.05).4.Cox single factor analysis showed that the higher the preoperative AFP,the lower the postoperative survival rate(P<0.05).ConclusionPreoperative serum AFP expression can well reflect the degree of differentiation of liver cancer cells, which can be used as an indicator of tumor malignancy. To evaluate the prognosis of liver cancer patients, preoperative detection of serum AFP concentration also has certain reference value.
