The Clinical Study Of Osteoarthritis After The Internal Fixation Of Acromioclavicular Joint Dislocation

Objective:To compare the curative effect of three kinds of the most commonly used internal fixation after acromioclavicular joint dislocation,and to observe and grade the postoperative complications of acromioclavicular joint osteoarthritis.Methods:Retrospect-ive analysis the 131 cases of Rockwood Ⅲ～Ⅴ acromioclavicular joint dislocation treated with internal fixation from 2011-03-2017-04,the modified Weaver-Dunn combined with clavicular hook plate(A group,60 cases),clavicular hook plate(B group,53 cases),double Endobutton plate(C group,18 cases).To compare the incident rate and severity of acromioclavicular joint osteoarthritis between the three groups in 1-3 years,and the Constant-Murley scores of shoulder function after the operation at the first,second and third year,And grading according to the severity of osteoarthritis.Results:131 cases were followed up in the three groups,the average follow-up time was 24(10～36)months.The incident rate of acromioclavicular joint osteoarthritis in group A(45.00%)was higher than that in group B(26.42%),while that in group B was higher than group C(5.56%),the difference was statistically significant(P<0.05).Group A was significantly higher than group B in the severity of acromioclavicular joint osteoarthritis,the difference was statistically significant(P<0.05).At the first year after operation,the Constant-Murley scores of shoulder joint function in group B were lower than those in group A and C,the difference was statistically significant(P<0.05).At the third years after operation,the Constant-Murley scores of shoulder joint function in group B and group C were significantly higher than those in group A(P = 0.032),the difference was statistically significant(P<0.05).Conclusion:The Modified Weaver-Dunn combined with clavicular hook plate internal fixation for the treatment of Rockwood Ⅲ～Ⅴ acromioclavicular joint dislocation is very easy to develop acromioclavicular osteoarthritis in the long term,and the incident rate and the severity of osteoarthritis is higher than the Clavicular hook plate and Double Endobutton plate fixation.
