| Objective To evaluate the clinical efficacy and safety of invigorating spleen qi in treating chronic atrophic gastritis with erosion(spleen deficiency and qi stagnation),and to explore its mechanism.Methods A total of 42 patients with chronic atrophic gastritis were diagnosed as chronic atrophic gastritis by endoscopy and pathological examination in the department of gastroenterology or physical examination of a hospital in wuhan.In the treatment group and the control group,randomized single-blind coding was performed in a 2:1 ratio,with 28 cases and 14 cases respectively.Before and after treatment,two groups of patients improved the safety examination(three routine,liver and kidney functions,electrocardiogram),and the treatment group was treated with oral and invigorating qi and blood stasis,and the control group was aluminum magnesium plus suspension.The patients were revisited once a week,and the syndrome differentiation was added and subtraction,and the score of the symptom scale was improved.1 course of treatment was conducted in 4 weeks.After the treatment,the patients were standardized to review gastroscopy and pathological tissue biopsy.The improvement of clinical symptoms,mucosal erosion and pathological tissue(chronic inflammation, atrophy and enterification)were compared between the two groups,and the drug safety assessment was conducted after treatment.Results(1)There was no statistically significant difference between the two groups in terms of gender,age,course of disease and clinical symptoms,gastroscope,and pathological score(P>0.05),and the data was comparable.(2)Clinical symptom evaluation of two groups of patients:(1)The total score of clinical symptoms was compared: before and after treatment of the two groups,the symptom score was lower than the previous one(P<0.05).Compared with the control group(P<0.01),the improvement of the treatment group was better than that in the control group(P<0.05).(2)In the treatment group,the symptom score of 2 and 4 weeks was lower than before,and the difference was significant(P<0.01;P<0.01);In the control group,there was a decrease in the 4 weeks and 2 weeks before treatment(P<0.05;P<0.05),there was no difference between the two groups(P > 0.05).For abdominal distention,than in the control group before and after treatment group,integral no difference(P>0.05),the treatment group after treatment points of successive down(P<0.05),compared with before and 4 weeks treatment group curative effect is better than that of control group(P<0.05).(3)Compared with the previous one,the score was decreased(P<0.05)compared with that before treatment in 4 weeks(P<0.05),and no difference was found in the control group(P>0.05).After the meal,the score of the treatment group was decreased(P<0.05),and the integral ratio was no difference between 2 and 4 weeks(P<0.05).In the control group,the fourth week of heartburn was lower than before treatment(P<0.05),and there was no difference in the full group after meal(P>0.05).(3)Endoscopic mucosal erosion evaluation: after treatment,the erosion score of both groups was lower than before treatment,and the difference was statistically significant(P<0.01;P<0.01),but there was no difference in comparison between groups after treatment(P>0.05).There were 24 cases and 11 cases in the treatment group and the control group,and the difference between the rank and the test was insignificant(P>0.05),indicating that the two groups had a similar effect on the improvement of mucosal erosion.(4)Histopathological evaluation: in the two groups,chronic inflammatory scores were compared,and the scores after treatment were lower than before treatment(P<0.01;P<0.05),but there was no significant difference between the two groups(P > 0.05),indicating that the two treatments had similar effects on the improvement of chronic inflammation.There was no significant difference between the two groups before and after treatment for atrophy and intestinal metaplasia(P>0.05).(5)Safety evaluation: during the treatment,there was no obvious adverse drug reaction and changes of biochemical indexes with clinical significance.Discussion It has good clinical effect on the patients with chronic atrophic gastritis accompanied by erosion(spleen deficiency and qi stagnation type).Through the holistic view and dialectical governance,this decoction can correct the imbalance between Yin and Yang of the body and the effect of Yin and Yang.To adjust the local mucosal environment to facilitate the repair of disease,to achieve comprehensive treatment,relieve symptoms,and improve the clinical efficiency of CAG with erosion.In the later stage,the treatment of CAG pathological tissue was improved,and the safety and side effects were low,and the clinical application could be popularized. |
