| Background Heart failure(HF)is a complex clinical syndrome which is cosed by serious stage of various heart disease,that is the leading cause of death in the elderly population of our country.Patients with heart failure show difficult breathing,fluid retention,atony and more clinical symptom and sign.How to slow the pathological process of heart failure,how to improve the clinical prognosis of patients with heart failure,t hese are focal point of research about heart failure.Astragalus can strengthen the body immunity,activate the mononuclear phagocyte system,reduce inflammatory damage,anti oxidative damage,boost metabolism,anti-tumor and more.Astragalus injection is a common form of preparation of astragalus in clinical.Objective Explorate the effect of astragalus injection on heart function indexes and myocardial injury indexes through animal experiments and clinical research to investigate what role does astragalus injection play in myocardium with heart failure.These study may provide theoretical and laboratory basis for clinical treatment of heart failure.Methods Animal experiments research methods:30 SD rats were randomly divided into 3 equal groups,these are astragalus group,model group and blank group.rats of astragalus group were injected 40mg·kg-1astragalus injection by peritoneal injection once a day for 7 weeks.On the eighth day,injected 2mL·kg~-11 adriamycin by peritoneal injection once a week for 6 weeks.After that,these rats were prohibited drinking water for 24h.Then the operation was done by routine.These rats were narcotized by 0.67%Pentobarbital Sodium by peritoneal injection,get 3ml venous blood from caudal vein.Brain Natriuretic Peptide(BNP)and n-terminal pro-b-type natriuretic peptide(NT-proBNP)in The 3ml venous blood were tested by enzyme linked immunosorbent assay(ELISA).Then different lead electrocardiogram(EGG)was recorded by multi-channel physiology recorder.Then the left carotid arteries were isolated for carotid artery intubatton,through the carotid artery intubatton to connect the pressure sensor of multi-channel physiology recorder,so the left ventricular systolic pressure(LVSP),the left ventricular end diastolic pressure(LVEDP)and±dp dtmax were recored.Finally these rats were killed to exposed their heart,cell microstructure of these left ventricular myocardium were observed by transmission electron microscope(TEM).As compared with astragalus group,astragalus injection was replace with isodose physiological saline,the rest of the operation of model group was same as astragalus group.As compared with astragalus group,astragalus injection and ADR were replace with isodose physiological saline,the rest of the operation of blank group was same as astragalus group.Statistical method:60 patients with heart failure were divided into 2 equal groups,these are astragalus group and model group.the patients of astragalus group were treated by conventional treatment that include cediland,diuretics,vasodilators,angiotension conversion enzyme inhibitor((ACEI),oxygen inhalation,dietary salt restriction,rest and more.Besides that,these patients were injected 30 mlastragalus injection once a day.One course of treatment is 10-14 days.As compared with astragalus group,the patients of model group were only treated by conventional treatment.After One course of treatment,left ventricular ejection fraction(LVEF),left ventricular end-diastolic dimension(LVEDD)and left ventricular end-systolic dimension(LVESD)were tested by echocardiography,and BNP and NT-proBNP in fasting venous blood from the patient′s elbow on morning were tested by ELISA.ResultsAnimal experiments research results:EGG of astragalus group and model group reflected myocardial injury,but myocardial injury of astragalus group was lesser than the injury of model group.LVSP and±dp dtmax of astragalus group was lower than those of blank group,and higher than those of model group;LVEDP of astragalus group was lower than that of model group,and higher than that of blank group;BNP and NT-pro BNP of astragalus group was lower than those of model group,and higher than those of blank group,these tesults are significantly different(P<0.05).Clinical research results:LVEF of astragalus group were higher than those of model group,LVEDD and LVESD of astragalus group were lower than those of model group,BNP and NT-pro BNP of astragalus group were lower than those of model group,these tesults are significantly different(P<0.05).there were lesser injure of cell membrane and organelle in myocardium cell microstructure of astragalus group,there were serious injury of of cell membrane and organelle in myocardium cell microstructure of model group.Conclusion Astragalus injection plays a protective role through reducing the structural damage of myocardial cells and myocardial damage index,on myocardium cells and and heart function with heart failure. |
