| Objective:To investigate the relationship between clinical characteristics of dyslipidemia in patients with primary IgA nephropathy and Lee’s pathological type and pathological type in Oxford.Methods:the renal biopsy confirmed primary IgA nephropathy patients with 266 cases,according to the lipid level into the normal lipid group and 93 cases of dyslipidemia group(173 cases,on the basis of total cholesterol total cholesterol,TC)were divided into high TC group and 54 cases of normal TC group 212 cases,on the basis of glycerin three greases(Triglyceride,TG)were divided into high TG group and 106 cases of normal TG group 1 60 patients were compared.Blood pressure,body mass index(Body Mass,Index,BMI),blood uric acid,blood urea nitrogen,serum creatinine,serum albumin,urine,24 h urinary protein,estimated glomerular filtration rate(estimated,Glomerular Filtration Rate,eGFR),analysis of the relationship between dyslipidemia and Lee’s pathological type and Oxford pathological types.Results:dyslipidemia group systolic blood pressure [(125.81 + 23.602)mmHg],diastolic blood pressure [(80.40 + 15.76)MMHg],BMI[(25.20 + 3.61)kg/m2],[79(64.00110.50)umol/L],serum creatinine,blood urea nitrogen [6.20(4.91,8.00)umol/L],blood uric acid[(347.24 + 98.12)umol/L],24 h urinary protein [(2.32 + 1.07)g/L] higher than the normal lipid group [(118.12 + 16.78 + 11.09(74.12)mmHg.)mmHg,(22.84 + 3.30)kg/m2,74.00(59.50,87.00)umol/L,4.10(5.50,7.55)umol/L,(307.69 + 80.06)umol/L,(1.70 + 0.86)g/L],eGFR[69.43(45.05-93.22)ml/(min-1.73m2)] is lower than the normal lipid group[86.71(70.09-106.80)mL/(min-1.73m2)](P < 0.05),2 groups of serum albumin,no statistically significant difference in the degree of hematuria(P > 0.05);dyslipidemia group Lee’s pathological type in the proportion of II-V [147(85)],Oxford branch ratio of[ interstitial fibrosis of renal tubular atrophy and / or between the type 173(65)] higherthan normal lipid group [67(72),93(35)](P < 0.05);group TG high shrinkage pressure,diastolic blood pressure,BMI,serum creatinine,blood urea nitrogen,uric acid,24 h urinary protein TG is higher than that of normal group,eGFR was lower than TG normal group(P < 0.05),the proportion of grade II-V Lees’ s pathological type,Oxford type renal tubular atrophy and / or Interstitial fibrosis was higher than that in normal TG group(P <0.05);high TC group BMI,serum uric acid,24 h urinary protein TC is higher than that of normal group(P < 0.05),the proportion of grade II-V Lees’ s pathological type,Oxford type renal tubular atrophy and / or interstitial fibrosis,mesangial cells hyperplasia,capillary hyperplasia,difference between segmental glomerulosclerosis ratio and TC normal group had no statistical significance(P > 0.05).The renal function of patients with IgA nephropathy conclusion:dyslipidemia than normal blood lipid,renal pathological damage is more severe for dyslipidemia group in high TG and high TC Patients with renal dysfunction,urinary protein,uric acid and other indicators increased,Lees classification,renal tubular / interstitial fibrosis more serious,early intervention,may reduce the occurrence of complications,delaying the progress of end-stage renal disease. |
