| Objective: To analyze the relationship between immunohistochemical markers P16,Ki-67 and Cyclin D1,and their relationship with Clinicopathological parameters,and to investigate the clinical prognostic significance;Combining the result of high-risk HPV infection,to analyze the relationship between HPV infection and clinical pathological parameters and immunohistochemical indexes and its clinical significance.Methods:(1)In our hospital,210 cases of squamous cell carcinoma of the head and neck were collected,all tissues were fixed by neutral formalin and paraffin embedded.(2)4μm four continuous slices,do HE staining and immunohistochemical En Vision two step method was used to detect the expression of P16,Ki-67 and Cyclin D1 proteins in tissues.(3)Clinical pathological and follow-up data were collected.(4)Using SPSS20.0 statistical software to analyze the data.Results: In head and neck squamous cell carcinoma(1)P16 positive expression rate was 38.10%,There were no relationship between the expression of P16 and sex,age,tumor grade,tumor T stage,lymph node metastasis and so on;Cyclin D1 high expression rate was 23.81%,The expression of Cyclin D1 was positively correlated with tumor grade(P=0.002);In Han,Uygur and other ethnic groups,high expression rate of Cyclin D1 increased gradually(P=0.015);Ki-67 high expression rate was 28.10%,The expression of Ki-67 increased significantly withthe increase of tumor grading,T staging,clinical staging,and lymph node metastasis(P=0.000,P=0.006,P=0.039,P=0.005);The expression of Ki-67 increased significantly in smokers(P=0.036).(2)No correlation between the expression of P16 with Cyclin D1 and Ki-67,and there was positive correlation between the expression of Cyclin D1 and Ki-67(r=0.211,P=0.002).(3)There was no significant difference in survival rate between P16+ and P16-(P=0.564);The expression of Ki-67 was negatively correlated with survival rate(P=0.010);Survival rate of Ki-67 high expressionand P16-was significantly lower than that of other cases(P=0.008);Survival rate of Cyclin D1 low expression was higher than that of Cyclin D1 high expression and moderate expression,but not significantly(P=0.152);Survival rate of Ki-67 low expression and Cyclin D1 low expression,moderate expression was higher than that in other cases(P=0.016);Survival rate of Ki-67 high expression and Cyclin D1 high expression,moderate expression was significantly lower than other cases(P=0.044).(4)HPV positive expression rate was 38.36%,The infection rate of HPV has nothing to do with age,smoking,ethnicity,anatomic site,tumor grade,clinical stage,P16,Cyclin D1 and Ki-67.The survival rate of HPV positive and HPV negative had no significant difference(P=0.718);In contrast,Survival rate of HPV+P16+ was higher than that of other cases(P=0.128).Conclusions: In head and neck squamous cell carcinoma(1)The expression of Cylcin D1 was positively correlated with the grading oftumor;The expression of Ki-67 was positively correlated with tumor grading,lymph node metastasis,tumor T staging and clinical staging.The expression of Ki-67 increased in smokers,and positively correlated with the expression of Cyclin D1.(2)The low expression of Ki-67 is a good prognosis;The prognosis of patients with high expression of Ki-67 and P16-was poor;The prognosis of Ki-67 low expression and Cyclin D1 moderate and low expression was good;When the high expression of Ki-67,the prognosis of Cyclin D1 moderate and high expression was poor;(3)HPV infection has nothing to do with prognosis. |
