Objective: By detecting the expression level of Treg cell-specific transcription factor Foxp3 and the inhibitory cytokine IL-10 in condyloma acuminatum lesions and subclinical infection lesions,and detecting HPV infection genotypes,we want to further investigate the Possible role of Treg cells in the cellular immunosuppression of condylomata acuminatum and subclinical infection and its association with HPV subtypes.Methods:Choose the patients of 101 patients with typical condylomata acuminatum who have never been treated in STD clinic of our department from July 2016 to July2017,30 SPI patients were diagnosed by clinical manifestations,ace to white test,histopathology examination and HPV-DNA detection to receive: 1.Group: 30 patients according to the typical condyloma lesions,condyloma acuminatum lesions,divided into typical condyloma acuminatum group,subclinical infection group,normal foreskin without HPV infection served as normal control group;2.PCR-reverse dot blot hybridization experiments was used to detect HPV types in typical CA group,subclinical infection group and normal control group;3.The expression of Foxp3 and IL-10 in typical condylomata acuminatum group,subclinical infection group and normal control group were detected by immunohistochemistry method.Results: 1.HPV-DNA test results:HPV-DNA was negative in 15 normal controls.The results of HPV-DNA test in 30 specimens of condyloma acuminatum were all positive,and10 kinds of HPV-DNA subtypes were detected in total of 47 specimens,including 13cases(27.66%)of HPV type 6 positive,8 cases(17.02%)of HPV type 11 positive,7 cases(14.89%)were positive for HPV-16,2 cases(4.26%)were positive for HPV-18,2cases(4.26%)were positive for HPV-39,3 cases(6.38%)were positive for HPV-52,2cases(4.26%)were positive for HPV-56,5 cases(10.64%)were positive for HPV-58,and 2 cases(4.26%)were positive for HPV-81.Among them,19cases(63.33%)were single infection,11cases(36.67%)were mixed infection,10 cases(33.33%)were high risk type and 20 cases(66.67%)were low risk type.The results of HPV-DNA test in 30 subclinical infection specimens were also all positive,and 9 kinds of HPV-DNA subtypes were detected in total of 42 specimens,including12 cases(28.57%)of HPV type 6 positive,7 cases(16.67%)of HPV type 11 positive,6 cases(14.29%)were positive for HPV-16,3 cases(7.14%)were positive for HPV-18,4 cases(9.52%)were positive for HPV-52,1cases(2.38%)were positive for HPV-56,4 cases(9.52%)were positive for HPV-58 and 2 cases(4.76%)were positive for HPV-81.21 cases(70.0%)were single infection,9 cases(30.0%)were mixed infection,11 cases(36.67%)were high risk type and 19 cases(63.33%)were low risk type.There was no significant difference in the frequencies of HPV positive type,low risk type,high risk type and single infection and mixed infection in the lesions of condyloma acuminata and subclinical infection(P> 0.05).2.Immunohistochemistry results:The mean optical density of Foxp3 protein expression in condyloma acuminata group,subclinical infection group and normal control group respectively were(0.201 ± 0.084)、(0.189 ±0.076)and(0.016 ± 0.020),The mean optical density of IL-10 protein expression respectively were(0.206 ± 0.091)、(0.199 ± 0.089)and(0.021 ± 0.005).The levels of Foxp3 and IL-10 in condyloma acuminata group and subclinical infection group were significantly higher than those in normal control group(P <0.05),but there was no significant difference between condyloma acuminata group and subclinical infection group(P> 0.05).The average optical density of Foxp3 in low-risk HPV and high-risk HPV respectively of condyloma acuminata group were(0.233 ± 0.081)and(0.177 ± 0.072),and the average optical density of IL-10 were(0.215 ± 0.080)and(0.167 ± 0.05);The average optical density of Foxp3 in low-risk HPV and high-risk HPV infection in subclinical infection group respectively were(0.191 ± 0.079)and(0.180 ± 0.062),and the average optical density of IL-10 were(0.207 ± 0.083)and(0.178 ± 0.05).The expression levels of Foxp3 and IL-10 in condyloma acuminata group and subclinical infection group were not significantly different from those in low and high risk type of HPV(P> 0.05).The average optical density of single and mixed infection Foxp3 in condyloma acuminata group respectively were(0.126 ± 0.013)and(0.296 ± 0.033),and the average optical density of IL-10 were(0.129 ± 0.017)and(0.285 ± 0.051);The average optical density of single and mixed infection of Foxp3 in subclinical infection group respectively were(0.112 ± 0.012)and(0.275 ± 0.052),and the average optical density of IL-10 was(0.135 ± 0.018)and(0.293 ± 0.065),The expression of Foxp3 and IL-10 in the condyloma acuminata group and subclinical infection group were statistically significant difference between single infection and mixed infection Significance(P <0.05).3.There was a positive correlation between the expression of Foxp3 and IL-10 in condyloma acuminata group and subclinical infection group(r = 0.873,P <0.01;r = 0.741,P <0.01).Conclusion: 1.The type of HPV detected in condyloma acuminatum group was HPV-6、-11、-16、-18、-39、-42、-52、-56、-58 and 81 type,and the type of HPV detected in skin lesions of subclinical infection group included HPV-6、-11、-16、-18、-42、-52、-56、-81,Both groups were mainly infectd by low-risk HPV-6、-11 and high-risk HPV-16、-58.There was no significant difference in the type of HPV infection between the two groups,indicating that there was no significant difference between the two groups in the type of HPV infection,although the clinical features were different,but there was no significant difference in the type of HPV-DNA between the two groups.2.No difference in low-risk or high risk HPV subtypes cause infection in the typical condyloma and subclinical infection,and the same as the single-infection and mixed infections differ.3.There was high expression of Foxp3 and IL-10 in the tissue of condyloma acuminatum and subclinical lesion,the expression level of Foxp3 and IL-10 were not related to low or high risk HPV infection,but were associated with single infection and mixed infection,indicating There is local cellular immune function suppression in condyloma acuminatum lesions and subclinical infection skin lesions.Foxp3 and IL-10 play an important role in the suppression of local cellular immune function,and local cellular immune function suppression in HPV-infected patients is more obvious suppressed than single HPV infection.4.The expression of Foxp3 and IL-10 was positively correlated with condyloma acuminatum and subclinical infection.
