| Objective: To investigate the changes of serum ferritin levels in patients withβ-thalassemia major before and after 7 years;to analyze the reasons of the increase in serum ferritin levels;and to Provide evidence and information to guide treatment.Methods: The survey was a retrospective questionnaire survey of 56 patients withβ-thalassemia major visited in 2010.Serum ferritin levels of patients was collected by a professional hematologist in the form of telephone follow-up and long-term follow-up of the We Chat group.and a comparative analysis based on data from 2010.Statistical methods: Quantitative data were statistically described by means of mean±standard deviation,count data is expressed as a percentage(n,%),Independent sample t-test was used for the comparison between the measurement data sets and the Pearson correlation analysis was used for the correlation analysis.The data was statistically analyzed using SPSS 25.0 statistical software,with P ≤ 0.05 being statistically significant.Results:The number of people in the survey was 56 cases,34 were males and 22 were females.The minimum age was 13 years and the maximum age was 30 years;the average age was 18.98±2.91.The male to female ratio was 1.5: 1.48 Of the 56 patients were still undergoing blood transfusion and iron chelation therapy.There were 29 males and 19 females with an average age of 18.63 ± 1.67.The data was relatively complete.Five patients had received hematopoietic stem cell transplantation and three patients died of heart failure.The data of 5 patients and 3 patients died were relatively incomplete.The questionnaire response rate is 85%.After 7 years of treatment,serum ferritin levels in 48 patients with β-thalassemia major who were still receiving blood transfusions and iron chelation therapy declined compared with 7 years ago,but there was no statistical difference between the two groups(P>0.05).There was no statistical relationship between cumulative transfusion time and serum ferritin(P>0.05).The serum ferritin levels of 3patients with diabetes was 4400±734.56μg/L.The serum ferritin levels of 5 patients without menstruation was 4620.47±527.49μg/L.The serum ferritin levels of 1 patient with amenorrhea was 3000 μg/L.Conclusion:1.After 7 years of treatment,the effect of iron chelation therapy in patients withβ-thalassemia major is not ideal,and the long-term quality of life and life expectancy in patients with β-thalassemia major are worrying.Health care workers need to strengthen the treatment and management of this group,and strengthen the patient’s regular iron chelation therapy awareness.2.Dysfunction of Pancreas and Gonad in patients with β-thalassemia major is closely related to elevated serum ferritin.Therefore,Regular monitoring of serum ferritin and timely adjustment of the treatment plan based on the test results are critical. |
