| Objective To study the impact of laparoscopic and open surgery on early prognosis of patients,to further explore the advantages and disadvantages of the two surgical approaches to guide theChoice of treatment options.Method Thirty-four Cases of Congenital Biliary Dilatation in our region were divided into laparoscopic group(n = 11)and operation group(n = 23)according to the operation method.By retrospective Case analysis,we analyzed the surgical methods,perioperative period Results,prognosis and follow-up Compared the advantages and disadvantages of this two kinds of surgery.Results Laparoscopic surgery was longer than laparotomy in open surgery group(290.91±64.45 min VS.213.04±55.04 min P= 0.001),and the amount of bleeding during operation was significantly less than that of open group(122.73±129.16 ml VS.367.39±365.75 ml,P= 0.040).There was a significant difference between the two groups after 3 days of pain score in laparoscopy group Compared with open group(2.27±0.65 VS.3.04±1.10P= 0.041).The postoperative pain in the Laparoscopic group was lighter than that in the open group.Laparoscopic group than the open group after the inflammatory response is small.(14.09±22.13 mg/l VS.34.46±28.68 mg/l),the difference was statistically significant(P=0.046).The length of postoperative hospital stay was 10.36±5.35 days in Laparoscopic group and 15.78±7.90 days in open group.The length of postoperative hospital stay was shorter in open group than in open group(P= 0.049).The overall complication rate was18.18% in the laparoscopic group and 34.78% in the open group,with no significant difference(P=0.437)between the two groups.Conclusion Although laparoscopic surgery lasted longer than laparotomy,it was superior to laparotomy in surgical bleeding,postoperative recovery,postoperative pain and appearance,and there was no significant difference in postoperative Complications between the two groups.Thus the laparoscopic radical surgery to treat CBD is safe,and with all these obvious advantages,it is worth promoting. |
