Objective:To investigate the quantitative parameters of dynamic contrast-enhanced MRI(DCE-MRI)in the evaluation of neoadjuvant chemotherapy in locally advanced breast cancer and metastatic lymph nodes.Methods: 42 patients who diagnosed with invasive ductal breast cancer underwent 3.0T DCE-MRI before and after neoadjuvant chemotherapy(NAC)in our hospital.Then,the maximum length,volumetric transfer constant(Ktrans),exchange rate constant(Kep),and extravascular extracellular volume fraction(Ve)were compared before and after the NAC.Result: All patients were divided into 4 groups with RECIST standard: complete remission(CR),partial remission(PR),stability(SD)and progress(PD).The mass disappeared completely in CR group after neoadjuvant chemotherapy.The values of Ktrans and Kep in PR group were significantly decreased after chemotherapy,and the value of Ve was slightly increased.There was statistically significant difference among the three groups before and after chemotherapy(P < 0.05).The Ktrans of SD group decreased slightly after chemotherapy and had statistical difference.Although the value of Kep decreased slightly,the value of Ve increased slightly,but there was no statistical difference in the groups.However,the values of Ktrans and Kep in PD group were increased to different degrees,and the difference before and after chemotherapy was statistically significant,The value of Ve decreased slightly,but the difference was not statistically significant.The difference of short diameter reduction before and after chemotherapy for metastatic lymph nodes in group A was statistically significant.The values of Ktrans and Kep decreased slightly after chemotherapy,the value of Ve increased slightly,but the difference were not statistically significant.There was also no significant difference in Ktrans,Kep and Ve values after chemotherapy in group B.Conclusion:The quantitative parameter of Ktrans,Kep and Ve has high evaluation efficacy in the evaluation of neoadjuvant chemotherapy for breast cancer,which is more significant in Ktrans and Kep.Ve value has good auxiliary value in assess the response to neoadjuvant chemotherapy for breast cancer.It can’t be used alone because it has more influence factors and less stability.To some extent,Ktrans,and Kep can also evaluate the anticancer effect of chemotherapy drugs on the axillary nodes,Supraclavian lymph node and Subclavian lymph node.
